ÖZET
Amaç: Şiddetli preeklampsi ve HELLP sendromu sonucu oluşan hepatik subkapsüler rüptür olgu sunumu. Çalışmanın Yapıldığı Yer: Hacettepe Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum Anabilim Dalı. Materyel ve Metod: Kliniğimizde 16.ıncı gebelik haftasında kronik hipertansiyon ve süperempoze preeklampsi tanısı sonucu yapılan tetkiklerde intrauterin exitus, HELLP sendromu ve akut karın saptanan ve bunu takiben planlanan eksploratif laparotomi ve histerotomi sonucunda karaciğerde kapsüler defekt saptanan anne. Bulgular: Akut karın nedeni ile yapılan acil laparatomi sonucunda subkapsüler hematoma bağlı olduğu düşünülen ön hepatik segmentte 2 cmlik bir kapsüler defekt saptandı. Defekt cerrahi olarak onarıldı. Yoğun bakım tedavileri sonucu postoperatif onuncu günde olgu taburcu edildi. Sonuç: Şiddetli preeklampsinin önemli komplikasyonlarından biri olan hepatik rüptür durumunda, her ne kadar ilk tanıyı koymada zorluk olsa da, zamanında teşhis ve erken tedavi maternal mortalite ve morbiditeyi azaltır. Bu nedenle hepatik rüptür olgularındaki güncel tanı ve tedavi yaklaşımları özetlenerek vurgulanmıştır.
ABSTRACT
Objective: A case report of hepatic subcapsular rupture due to severe preeclampsia and HELLP sendrome. Institution: Hacettepe University School of Medicine, Department of Obstetrics and Gynecology. Material and Method: A mother who had been diagnosed at a gestational age of 16 weeks due to a fetal demise, severe preeclampsia and HELLP syndrome followed by an explorative laparotomy and hysterotomy revealing hepatic capsular defect. Results: An emergency laparotomy undertaken due to acute abdomen revealed a 2 cm capsular defect on the anterior hepatic surface thougth to be secondary to subcapsular hematoma rupture. The defect is surgically packed. The case is discharged from the hospital on the 10th. day following the operation and intensive care therapies. Conclusion: Prompt recognition and early treatment of hepatic rupture which is one of the important sequelae of severe preeclampsia, may reduce the maternal mortality and morbidity, although the diagnosis may be initially confusing. Thus, new trends in diagnostic and therapeutic approaches in hepatic rupture cases are outlined and reviewed.
