ÖZET
Anormal uterin kanamalar, kadın doğum kliniklerine başvuruların en önemli nedenlerinden biri olmakla birlikte henüz evrensel olarak kabul görmüş bir terminoloji bulunmamaktadır. Kanama nedeni olabilecek sebeplerin tüm klinisyenler tarafından kullanılan ortak bir sınıflamasının olmayışı veya bir hastada birden fazla olası nedenin varlığı gerek klinisyenler arasında gerekse literatürde konu hakkında fikir birliği sağlanmasını engellemektedir. Bu eksikliği gidermek amacıyla 2011 yılında 6 kıta ve 17'nin üzerinde ülkeden araştırmacıların desteği ile International Federation of Gynecology and Obsterics (FIGO) Menstrual Disorders Group (FMDG) tarafından reprodüktif çağda gebe olmayan kadınlarda anormal uterin kanamalarda kullanılan terminolojiyi standardize etmek amacıyla PALM-COEIN sınıflaması (polyp, adenomyosis, leiomyoma, malignancy and hyperplasia, coagulopathy, ovulatory dysfunction, endometriyal, iatrogenic, not yet classified) oluşturulmuştur. PALM yapısal nedenleri gösterirken COEIN ise yapısal olmayan nedenleri tanımlamaktadır. Amaç, olası nedenlerin, zaman içerisinde meydana gelecek gelişmeler sonrası yapılabilecek adaptasyonlara da izin verecek şekilde, ortak bir terminoloji ile gruplanmasıdır. Yeni sınıflama sistemi ile FIGO, onkolojik evrelemelerde olduğu gibi anormal uterin kanamada da pratik ve evrensel bir sınıflama sistemi geliştirmeyi hedeflemiştir. ''American College of Obstetricians and Gynecologists'' tarafından da kabul gören sınıflama ile bundan sonra konu hakkında yapılacak çalışmaların meta-analizine olanak sağlanması, yeni tedavilerin araştırılmasına ışık tutması hedeflenmektedir. Bu derlemede, anormal uterin kanamalarda FIGO tarafından geliştirilen PALM-COEIN sınıflaması detayları ile anlatılamaktadır.
ABSTRACT
Altough abnormal uterine bleeding is one of the major complaints in the admission to obsterics and gynecology clinics, there is still an inconsistency in the terminology. Absence of a consensus classification about the possible causes of abnormal uterine bleeding or involvement of more than one possible mechanism hamper the clinicians to reach an agreement. To address this issue, with the contribution of investigators from 6 continents and over 17 countries International Federation of Gynecology and Obsterics (FIGO) Menstrual Disorders Group (FMDG) proposes a new system PALM-COEIN (polyp, adenomyosis, leiomyoma, malignancy and hyperplasia, coagulopathy, ovulatory dysfunction, endometrial, iatrogenic, not yet classified) for classification of abnormal uterine bleeding. The PALM group describes the structural causes while the COEIN describes nonstructural ones. The goal of the classification is to develop a common terminology that is capable of adaptation to the growing evidence about the mechanisms of abnormal uterine bleeding. With this new system FIGO intented to form a practical and universally accepted classification system as FIGO oncology staging systems. This classification system, which is also accepted by American College of Obstetricians and Gynecologists, will foster meta-analysis of new clinical trials and help in the investigation of new treatments. In this review the PALM-COEIN classsification is described in details.
