ÖZET
Amaç: Adneksiyal kitle ile başvuran hastalarda trombositozun, ultrasonografik görünüm ve serum CA-125 değerleri ile birlikte malignitenin preoperatif teşhisinde önemi olup olmadığının araştırılması. Çalışmanın Yapıldığı Yer: Ondokuz Mayıs Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum Anabilim Dalı, Samsun. Materyel ve Metod: Adneksiyal kitle ile başvuran 80 hastada preoperatif olarak trombosit sayısı, serum CA-125 seviyeleri ve ultrasonografik (USG) görünüm değerlendirildi. Trombosit sayısının 350.000/mm3 üzerinde, CA-125 değerinin 35U/ml üzerinde olması; USG için ise skorun 9 ve daha yüksek olması malignite ile uyumlu kabul edildi. Postoperatif histopatolojik tanıya göre, her bir tetkikin ayrı ayrı ve kombine olarak sensitivite, spesifisite, pozitif (+) ve negatif (-) prediktif değerleri hesaplandı. İstatistiksel analiz için c2 testi kullanıldı. Bulgular: Histopatolojik inceleme sonrası 80 hastadan 56'sı benign, 24'ü malign olarak sınıflandırıldı. Trombositozun malign-benign ayırımı yapabilmedeki sensitivitesi %54,2, spesifisitesi %78,6, (+) prediktif değeri %52, (-) prediktif değeri %80 bulundu. Sensitivitesi en yüksek olan tetkik USG (%87,5), spesifisitesi en yüksek olan tetkik CA-125'ti (%80,4). Üç test kombine edildiğinde sensitivitenin %91,7'ye, (-) prediktif değerin %95,1'e çıktığı görüldü. Sonuç: Trombosit sayısı adneksiyal kitlelerin preoperatif ayırıcı tanısında tek başına yeterli bir tetkik olmamakla birlikte, serum CA-125 ve USG ile kombine edildiğinde bu tanı yöntemlerinin sensitivite ve (-) prediktif değerlerini arttırmaktadır.
ABSTRACT
Objective: To determine the significance of thrombocytosis together with ultrasonographic appearance and serum CA-125 levels, in the preoperative diagnosis of malignant adnexal masses. Institution: Ondokuz Mayıs University Faculty of Medicine, Department of Obstetrics and Gynecology, Samsun. Materials and Methods: Eighty patients with adnexal mass were preoperatively evaluated by thrombocyte count, serum CA-125 levels and ultrasonography (USG). Thrombocyte count >350000/mm3, CA-125 levels >35U/ml and USG score ³ 9 were considered to be associated with malignancy. According to the postoperative histopathologic diagnoses, sensitivity, specificity, positive (+) and negative (-) predictive values for each test and the combination of the tests were calculated. c2 test was used for statistical analysis. Results: After histopathologic evaluation, 56 out of 80 patients were classified as benign and 24 were classified as malignant cases. Thrombocytosis had a sensitivity of 54.2%, specificity of 78.6%, (+) predictive value of 52% and (-) predictive value of 80% in the determination of malignancy. The most sensitive test was USG (87.5%) and the most specific test was CA-125 (80.4%). When all three tests were combined, the sensitivity and (-) predictive values were increased up to 91.7% and 95.1% respectively. Conclusion: Although thrombocyte count by itself is not so significant in the differential diagnosis of adnexal masses; when combined with serum CA-125 and ultrasonographic examination, it increases their sensitivity and the (-) predictive values.
