Alfentanil Propofol Anaesthesia In Minor Gynecological Procedures: A Comparative Study With Propofol

ÖZET
Amaç: Bu çalışma, spontan solunumlarına bırakılan, küçük jinekolojik girişim geçirecek poliklinik olgularında propofol ile, alfentanil-propofol kombinasyonunun anestetik etkinliğini ve derleme özelliklerini karşılaştırmak amacıyla yapıldı. Çalışmanın Yapıldığı Yer: Gazi Üniversitesi Tıp Fakültesi, Anesteziyoloji AD, Ankara. Materyel ve Metod: Yasal tahliye ve probe kürtaj geçirecek ASA I-II grubundan premedikasyonsuz 44 kadın olgu iki eşit gruba ayrıldı. Tüm olgulara antiemetik olarak 0.0175 mg/kg droperidol verildikten sonra I. gruba propofol 2 mg/kg, II. gruba alfentanil 8 mikrogr/kg ve propofol 1.5 mg/kg iv verilerek indüksiyon yapıldı. Anestezi idamesi gerektikçe verilen 20 mg'lık bolus propofol ile sağlandı. Bulgular: Gruplar arasında cerrahi girişim süreleri açısından fark yoktu. Alfentanil'in total 447.72 ± 160.71 mikrogr verildiği grupta gerekli propofol miktarı toplam 132.20 ± 51.10 mg iken, pür propofol grubunda 173.41 ± 63.61 mg olup, alfentanil uygulanan grupta propofol dozunda azalma önemli idi (p< 0.05). Pür propofol kullanılan grupta ortalama arter basıncında 2. ve 5. dakikalarda önemli düşüş sağlandı. Sonuç: Propofol'e küçük doz alfentanil ilavesi ile, daha yeterli anestezi, daha kısa (p< 0.05) ve daha kusursuz derlenme dönemi sağlandığı sonucuna varıldı.
ABSTRACT
Objective: This study was performed to investigate the anaesthetic efficiacy and the properties of recovery of propofol and combination of alfentanil-propofol in patients undergoing minor gynecological surgery, who were breathing spontaneously. Institution: Gazi University School of Medicine, Department of Anaesthesiology and Reanimation, Ankara. Material and Methods: Unpremedicated 44 female patients in ASA I-II group, undergoing dilatation and curettage were divided into two identical groups. Following the administration of 0.0175 mg/kg droperidol as an antiemetic for both groups, first group received alfentanil 8 microgr/kg and propofol 1.5 mg/kg for the induction of anaesthesia. The maintenance of anaesthesia was provided with the bolus doses of propofol 20 mg when necessary. Findings: There was no difference in the duration of surgery between the groups. While alfentanil administered group in doses of 447.72 ± 160.71 microgr totally required propofol 132.20 ± 51.10 mg doses, only propofol administered group required propofol dose of the alfentanil administered group was found to significant (p< 0.05). In the pure propofol administered group, significant decrease at the second and fifth minute in the mean arterial pressure was determined (p< 0.05). Results: It was concluded that small doses of alfentanil supplementation to propofol provided more satisfactory, shorter and more uneventful recovery.
