ÖZET
Amaç: Uterusun benign lezyonlarında histerektomi öncesi yapılan dilatasyon ve küretajın (D&C) gerekliliğinin incelenmesi. Çalışmanın Yapıldığı Yer: Gazi Üniversitesi Tıp Fakültesi, Kadın Hastalıkları ve Doğum Kliniği. Materyal ve Metod: Ocak 1990-Ekim 1995 tarihleri arasında myoma uteri, kronik pelvik ağrı tanısı alan 366 hastaya histerektomi öncesi D&C yapılıp, elde edilen bulgular operasyon sonrası histerektomi materyalinin patolojik inceleme sonuçları ile karşılaştırıldı. Bulgular: Preoperatif yapılan D&C sonuçlarında, 226 vakada proliferatif endometrium, 95'inde sekretuar endometrium, 28'inde kistik glandular hiperplazi, 9'unda adenomatöz hiperplazi, 3'ünde atipik hiperplazi, 5 vakada ise atrofik endometrium tesbit edilirken hastaların hiçbirinde malignite tesbit edilmedi. Postoperatif spasimen incelemesinde sadece bir vakada D&C'de tesbit edilemeyen atipik endometrial hiperplazi bulundu. Sonuç: Myoma uteri, kronik pelvik ağrı gibi benign nedenlerden dolayı histerektomi planlanan hastalara, tedavi şeklini değiştirmediği için rutin preoperatif D&C'a gerek olmadığı sonucuna varılmıştır.
ABSTRACT
Objective: To investigate the value of prehysterectomy endometrial curettage in patients with benign uterus lesions (myoma uteri, chronic pelvic pain). Institution: Gazi University Faculty of Medicine, Department of Obstetrics and Gynecology, Ankara. Material and Methods: Between January 1990-October 1995, 366 women with benign uterine lesions have undergone dilatation and curettage (D&C) prior to hystrectomy. After the operation, the results of D&C have been compared to hysterectomy specimen obtained. Results: D&C results revealed that 226 cases with proliferative endometrium, 95 cases with secretory endometrium, 28 cases with cyctic glandular hyperplasia, 9 cases with adenomatous hyperplasia and 3 cases with atrophic endometrium. Conclusion: We conclude that routine D&C prior to hysterectomy in cases with benign uterine lesions is not necessary, since D&C has changed treatment protocol in none of the cases.
