Birth Method Choices Of Primipar Women And The Factors Which Have An Effect On These Choices

ÖZET
Amaç: Bu çalışma bir kez doğum yapmış kadınların doğum şekli ile ilgili tercihlerinin belirlenmesi ve doğum şeklinin tercihinde etkili olan faktörleri incelemek amacıyla yapıldı. Gereç ve Yöntemler: Araştırmamıza, Mart-Eylül 2006 ayları arasında Afyonka-rahisar il merkezinde yataklı tedavi hizmeti veren tüm hastanelerde ilk kez doğum yapan kadınlar alındı. Araştırmamıza katılmayı kabul eden ve bilgilendirilmiş onam formu doldurulan 314 gebe çalışmaya dahil edildi. Çalışmamız kesitsel tipte dizayn edildi. Veri toplama aracı olarak, 43 sorudan oluşan anket formu kullanıldı. Verilerin analizinde tanımlayıcı ölçütler (yüzde, minimum, maximum, v.b.) korelasyon analizi ve ki-kare test kullanıldı, anlamlılık seviyesi olarak p<0.05 alındı. Bulgular: Araştırmaya katılan kadınların %77.4'ünün vajinal doğumu, %22.6'sı sezaryen doğumu tercih ettiği saptandı. Kendi isteği ile sezaryen olan kadın oranı ise % 6.8 bulundu. Bir önceki doğum şeklinin sezaryen doğum olmasının kadınların sezaryen tercihi üzerinde anlamlı artışa neden olduğu görüldü. Doğum şekli tercihinde kadınların eğitim durumunun, gebelik öyküsünün, gebeliğin takip edildiği yerin, gebeliği takip eden kişinin, doğumun gerçekleştiği yerin etkili olmadığı saptanırken, doğumu gerçekleştirecek kişinin doktor olması, doktorun cinsiyetinin erkek olması, ekonomik durumun yükselmesi ile sezaryen tercihinin arttığı saptandı. Kadınların doğum şekilleriyle ilgili olarak yeterli bilgilendirilmedikleri tespit edildi. Sonuç: Çalışmamızda sezaryen tercihinin istemli sezaryen olma oranlarıyla kıyaslandığında yüksek saptanması, son yıllarda artan sezaryen oranlarında kişisel tercihin önemini ortaya koymaktadır. Ülkemizde artan sezaryen oranları göz önüne alındığında doğum tercihine etki eden faktörlerin bilinmesi ve gerekli önlemlerin alınması oldukça önemlidir.
ABSTRACT
Objective: This study has been carried out to determine the choices of women who have given their first birth with regard to birth method and to look into the factors affecting these choices of birth method. Material and Methods: This study has been applied to all the patients who were willing to participate and who gave their first births in the maternity wards of hospitals at Afyonkarahisar without choosing particular specimens. 314 patients who were willing to participate and filled informed consent form were included in the study. A cross-sectional study design was used. A survey comprising of 43 questions was used as a data gathering device. Descriptive criteria (percent, minimum, maximum etc.), correlation analysis and Fischer test were used in the data analysis process and p<0.05 was determined as the level of significance. Results: At the end of the study 74.0 % of the women opted for vaginal birth while 22.6% of them chose caesarian method of birth. When we look into the reasons for caesarian method, the percentage of the woman who have this kind of birth on their own will is 6.8%. The fact that their previous birth methods were caesarian had a significant effect on women's choosing caesarian method. When we look into the reasons of choosing one of these methods, there were no significant difference with regard to their level of education or pregnancy history, location of the pregnancy follow-up, but there was a significant difference between the birth method choice and their levels of income. In addition to these, doctors, especially males, were seen to have an important role in the decision of birth method. It is noted that the women participating in the study weren't properly informed about birth methods. Conclusion: In our study, cesarean section on demand increases the rates of cesarean section. High rates of caserean section in our country shows that factors effect the birth method choices should be known and necessary preventions should be made.
