Changes In Hormonal Parameters With The Long Term Use Of Gn-RH Agonist Treatment
Endometrioisis Nedeniyle Uzun Süreli İntranazal Buserelin Tedavisi Sırasında Gözlenen Hormonal Parametrelerdeki Değişiklikler

ÖZET
Amaç: Uzun süreli Gn-RH agonist ilaç kullanımı ile hormonal değerlerde oluşan değişiklikleri incelemek. Çalışmanın Yapıldığı Yer: Dr. Zekai Tahir Burak Kadın Hastanesi, Ankara. Materyal ve Metod: Endometriosis nedeniyle 6 ay süreyle 1200 mg/gün dozda intranasal Gn-RH agonist (Buserelin) kullanılan 29 hastanın serum Follikül Stimulan Hormon (FSH), Luteinizan Hormon (LH), Estradiol (E2) ve prolaktin (PRL) düzeylerindeki değişiklikler incelendi. Bulgular: FSH değerlerinde tedavi öncesine göre tedavi süresince belirgin bir farklılık gözlenmedi (p> 0.05). LH değerleri tedavinin 2. ayından itibaren suprese oldu (p= 0.001). Estradiol ve prolaktin değerleri de LH ile korele bir şekilde tedavinin 2. ayından itibaren suprese oldu (E2: p= 0.0001; PRL p= 0.0001). Sonuç: Uzun süreli Gn-RH agonist tedavisinde özellikle suboptimal doz kullanıldığında estradiol supresyonu gonadotropin hormonlarla korele seyretmemektedir. Gonadotroplardaki bioaktif ve immunaktif kompenentlerdeki değişme, uygulanan doz ve Gn-RH agonistlerin hipofiz dışındaki etkiler bu durumu açıklayabilir. Dolayısıyla Gn-RH analogu ile tedavi edilen hastalarda tedavi etkinliği serum estradiol düzeyleri ile takip edilmelidir.
ABSTRACT
Objective: To evaluate the changes in hormonal parameters with the long term use of Gn-RH agonist treatment. Institution: Dr. Zekai Tahir Burak Woman's Hospital, Ankara. Material and Methods: During monthly controls, changes in follicle stimulating hormone (FSH), luteinizing hormone (LH), Estradiol (E2), and prolactin (PRL) concentrations were evaluated of 29 patients who received a 6 month course of intranasal (Gn-RH) agonist therapy (Buserelin) for endometriosis. Results: No significant change was observed in FSH throughout the treatment period (p> 0.05). LH levels decreased significantly in the second month of treatment (p= 0.001). E2 and PRL levels were also suppressed correlated with LH (E2: p= 0.0001, PRL p= 0.0001). Conclusion: These results show that during long-term Gn-RH agonist treatment, suppression in estradiol does not correlate with gonadotrophin hormones, especially when suboptimal doses are used. Changes in bioactive and immunoactive components of gonadotrophins, dose used, and the extrapituitary effect of Gn-RH agonists are the possible causes of this observation. There fore we conclude that estradiol value is the only reliable parameter in monitoring the effectiveness of Gn-RH agonist therapy.
