ÖZET
Amaç :GTH(Gestasyonel trofoblastik hastalık) tanısı alan 165 olgunun, klinik özeliklerini,tedavi yaklaşımlarını ve sonuçlarını araştırmak ve bu bulguları literatür ile karşılaştırmak.Materyel Metod : Zekai Tahir Burak Kadın Sağlığı Eğitim ve Araştırma Hastanesi Onkoloji Kliniğinde, 1996-1999 yılları arasında GTH tanısı ile takip edilen 165 hastanın dosyaları tanı, tedavi ve sonuçları bakımından retrospektif olarak incelendi.Bulgular: 165 olgunun 100ü(%60.6) mol hidatidiform, 45i (%27.2) persistan mol ve 20si (%12.1) malign GTH tanısı almıştı.GTH sıklığını 1000 doğumda 1.78, koryokarsinom sıklığını 1000 doğumda 0.21 olarak bulduk.Her üç gruptaki hastaların yaş dağılımları arasında anlamlı bir fark mevcuttu (p<0.05). Toplam 10 hastada (%6) geçirilmiş mol hidatidiform öyküsü mevcuttu. Hastaların %71.5i kliniğe vajinal kanama ile başvurdu, %20sinde bulantı-kusma mevcuttu. 100 MHlu(Mol hidatidiform) olgunun 96sı dilatasyon ve vakum ile kürtaj ile tedavi edilirken, sadece 4 hastaya ileri yaşta olmaları nedeniyle total abdominal histerektomi uygulandı. Persistan mol olgularında ilk adım olarak tek ajan kemoterapi uygulandı. Bu grupta 40 hasta (%89) metotraksat ile tam bir remisyon sağlandı.Bu grupta kür sayısı 1-9 arasında değişmekteydi. (Ortalama 3.6 kür). Tek ajan kemoterapisine yanıt vermeyen 5 hastadan 3ünde (%6.6) metotraksat ve aktinomisin-D kullanıldı ve 2 hastaya (%4.4) multiajan kemoterapi protokolünden EMA-CO(etoposide, methotraksat, aktinomisin-c ve siklofosfamid, vinkristin) uygulandı. Malign GTH olgularında, hastaların FIGO evrelemesine göre dağılımı şu şekildeydi: 14 hasta (%70) IA, 4 hasta (%20) IB, 2 hasta (%10) IIIA evresinde idi.Bu grupta 12 hastaya Mtx.,6 hastaya Mtx.+ Act.-D ve 2 hastaya EMA-CO uygulandı. 2 olguda ve perfarasyon nedeniylede bir olguda toplam 3 olguda total abdominal histerektomi yapıldı.Sonuç: Tedavi öncesi ?hCG değerlerinin yüksek olması ve remisyon için geçen sürenin (bhCG düzeyinin negatif olması) uzamasının, persistant hastalık ve malignite riskini artırdığı gözlendi. Erken teşhis ve tedavi ile başarı oranının %100e ulaştığı bir hastalık olan GTHta, hastanın yakından takibi, tedavisinin en önemli kısmını oluşturur. Remisyonu takiben de, hastalar kaderine terkedilmemeli ve hastalığın önemi ve tehlikeleri hastaya etraflıca anlatılmalıdır.
ABSTRACT
Aim: The aim of the study was to analyze the clinical characteristics, treatment approaches and outcome of 165 patients with gestational trophoblastic disease and compare the results with the literature.Material and Method: One hundred sixty five (165) cases diagnosed as GTD was admitted to Zekai Tahir Burak Women Health Education and Research Hospital between 1996-1999.All patients were evaluated retrospectively according to their clinical characteristics, treatment modalities and outcomes in Oncology department.Findings: Out of 165 cases, 100 was diagnosed as mole hydatidiform (60.6%), 45 was diagnosed as persistan mole (27.2%) and 20 as malign GTD (12.1%). The prevalence of GTD and choriocarcinoma were 1.78 and 0.21 in 1000 birth respectively. There was a significant difference (p<0.05) regarding the ages of the three groups. A history of previous hydatidiform mole was found in 6% of the patients. The most common presenting symptom was vaginal bleeding with 71.5% and nausea-vomiting with 20%. Dilatation and vacuum curettage was the first line treatment for 96 patients with mole hydatidiform but only 4 patients were treated by total abdominal hysterectomy because of older age. The persistant cases fırstly were treated by single agent methotraxate chemotherapy. Complete remission was achieved with the administration of methotrexate in 40 patients (89%). In this group the number of the chemotherapy cycle was changing between 1-9 (On average 3.6) In 5 patients who were resistant to single agent methotrexate chemotherapy, 3 of them (6.6%) were treated with combination of methotrexate and actinomisin-D and 2 of them (4.4%) were treated with multiagent chemotherapy EMA-CO. In malign GTD group 14 patients (70%) were in Stage I A, 4 patients (20%) were in Stage IB and 2 patients (10%) were in Stage IIIA according FIGO staging system.12 patients received Mtx., 6 patients received Mtx.+ Act-D and 2 patients received EMA-CO. (etoposide, methotrexate, actinomycin D and cyclophosphamide, vincristine). In 2 Stage III A cases and in one uterine perforation case, total abdominal hysterectomy was performed.Conclusion: It was observed that when the ßhCG levels were high before the treatment and in case of a longer period of remision, the malignancy and persistant disease risk was increased. In GTD, with early diagnosis and treatment, 100% cure can be succeeded and close follow up of these patients is the most important point of the treatment. The importance and the risks of the disease must be explained to the patients and these patients must be followed after the remision.
