ÖZET
Amaç: Bu çalışma ile preoperatif verilen progesteron ve GnRh analoglarının postoperatif adezyonları önlemedeki etkinliklerinin araştırılması ve karşılaştırılması amaçlandı. Çalışmanın Yapıldığı Yer: Haydarpaşa Numune Eğitim ve Araştırma Hastanesi Hayvan Laboratuvarı Yöntem: Her biri 12 wistrar sıçanından oluşan üç grup oluşturuldu. I. grup: İlk operasyondan üç hafta önce 15 mg intramuskular (i.m.) medroxyprogesteron asetat (MPA) verildi. II. grup: İlk operasyondan yedi gün önce GnRh analoğu Leuprolid acetat 0,75 mg intramuskular (i.m.) verildi. III. grup: Kontrol grubu Tüm wistrar sıçanlara aynı kişi tarafından müdahale yapıldı. Postoperatif üçüncü haftada ikinci bir müdahale ile adezyonlar skorlanarak 0-3 arası değerlendirildi ve uzunlukları ölçüldü. Bulgular: Preoperatif MPA uygulamasının postoperatif adezyonları azaltmada GnRH (skor: p<0,01, uzunluk: p<0,05) ve kontrol (skor: p<0,001, uzunluk, p<00,01) gruplarına göre belirgin olarak etkili olduğu görüldü. GnRh analoglarının etkisi kontrol grubuna göre istatistiksel olarak anlamlı bulunmadı (p>0,05). Sonuç: MPAnın preoperatif i.m. verilmesi adezyon formasyonunu belirgin olarak azaltır. GnRh analoglarının adezyonları önlemedeki etkisi istatistiksel olarak anlamlı değildir.
ABSTRACT
Objective: In this study we aimed to investigate the effects of preoperative administration of progesterone and GnRh analogues in prevention of postoperative adhesions. Institution: The study was conducted in Haydarpaşa Numune Educational Research Hospital in Istanbul. Study Design: Wistrar rats were randomly divided into groups, each group containing 12 wistrar rats: Group I: Was given 15 mg medroxyprogesterone acetate (MPA) intramuscularly 3 weeks before the first laparotomy. Group II: Was given leuprolide acetate 0,75 mg intramuscularly 7 days before the first laparotomy. Group III: Was the control group. All the wistrar rats were operated by the same surgeon. On the postoperative 3rd week they were explored for adhesions which were scored 0-3. Results: Preoperative administration of MPA was more effective in preventing postoperative adhesions in comparison to GnRh analogues (grade: p<0,01, adhesion length: p<0,05) and controls (grade: p<0,001, adhesion length: p<0,01). The effect of GnRh analogues in comparison to the control group in preventing adhesion was statistically not significant (p>0,05). Conclusion: Preoperative administration of MPA is highly effective in preventing postoperative adhesions in the animal model. The effect of GnRh analogues is not statistically confirmed.
