Comparison Of The Metabolic Consequences Of Transdermal Estradiol Replacement Therapy Alternatives In Postmenopausal Patients

ÖZET
Amaç: Hormon replasman tedavisi uygulanan postmenopozal olgularda transdermal östradiol jel ile östradiol salgılayan patch şeklindeki östrojen replasman tedavisi seçeneklerinin etkinlik ve güvenilirliğinin karşılaştırılması.Çalışmanın Yapıldığı Yer: SSK Ankara Doğumevi ve Kadın Hastalıkları Eğitim Hastanesi.Materyal ve Metod: Hormon replasman tedavisi planlanan 62 postmenopozal kadın çalışmaya dahil edilerek bir yıl süre ile randomize olarak yarısına transdermal östradiol jel (3x 0.5 mg/gün, Estreva®), diğer yarısına da transdermal patch (50 µg östradiol / 24 saat, Estraderm TTS®) şeklinde östrojen replasman tedavisi uygulandı. Tedaviye her ayın son 12 günü, günde 200 mg doğal mikronize progesteron (Progestan ®) eklendi. En az altı ay süreyle uygulanan tedavinin üçüncü ve altıncı ayında ilaçların klinik semptomlar, endometrium kalınlığı, hormon profili, lipid metabolizması (HDL, LDL, trigliserid, kolesterol) ve karaciğer enzimleri (AST, ALT) üzerine olan etkileri ve ilaca bağlı sorunlar, kullanım kolaylığı ve hasta memnuniyeti araştırıldı. Bulgular: Her iki grupta olguların ortalama yaşları, menopoz süreleri ve daha önce hormonal tedavi kullanma oranları benzerdi. İlaca bağlı yan etki (lokal deri reaksiyonu) gelişen olgu sayısı patch kullanılan grupta istatistiksel anlamlı oranda fazla bulundu (Jel kullanılan grupta %29, n=9 ve patch kullanılan grupta %67, n=21; p=0.002). Her iki grupta bazal FSH ve LH değerlerinde üçüncü ayda ve altıncı ayda bazal değerlere göre kademeli ve anlamlı düşüş olduğu, östradiol seviyelerinin ise bazal değerlere göre kademeli ve anlamlı olarak yükseldiği saptandı (p<0.001). Her iki grupta bazal değerler ile altıncı ayda ölçülen değerler karşılaştırıldığında HDL seviyelerindeki artış anlamlı idi (Jel grubu için p<0.001 ve patch grubu için p=0.004). Jel grubunda diğer lipid profili parametrelerinde bazal değerlere göre altıncı ayda istatistiksel anlamlı düşüş saptanırken, patch grubunda LDL seviyelerinde anlamlı düşme izlenmedi. Jel grubunda bazal değerlere göre üçüncü ay ALT değerinde anlamlı düşüş saptanırken, AST düzeylerinde her iki grupta anlamlı değişme saptanmadı (Jel grubunda ALT için p=0.006).Sonuç: Patch ve jel şeklinde uygulanan tedavilerin her ikisi de benzer metabolik etkinlikte yöntemler olmasına karşın jel şeklindeki tedavide hasta memnuniyetini ve tedaviyi bırakma oranını etkileyen bir faktör olan lokal deri reaksiyonu daha az görülür. Jel şeklindeki tedavinin kullanımı daha kolaydır ve tedaviden hasta memnuniyeti daha fazladır.
ABSTRACT
Objective: To compare the efficacy and safety of alternatives of hormone replacement therapy in the form of transdermal estradiol gel or patch in postmenopausal women.Institution: SSK Ankara Maternity Womens Health Teaching Hospital.Materials and Methods: Sixty-two postmenopausal women who were planned to receive hormone replacement therapy were included in the study. Thirty-one women were given transdermal estradiol gel (3x 0.5 mg/ day, Estreva ®), the other half received transdermal estradiol patch (0.05 mg/day, Estraderm ®) for one year. All patients received an additional micronized progesterone (200 mg/ day, Progestan ®) daily during the last 12 days of the treatment cycle. The effect of this therapy on clinical symtoms, endometrial thickness, hormonal profile, lipid metabolism and liver function tests were evaluated in the third and sixth months and compared with the basal values. Patient pleasure and problems related with the use of patch or gel form were also questioned in both groups.Results: Mean age of women, duration of menopause and incidence of previous hormone replacement therapy use were similar in both groups. Percentage of women who had a local skin reaction were higher in the transdermal patch group (29% in the gel group, 67% in the patch group; p=0.002). In both groups, there was a gradual and statistically significant decrease in FSH and LH levels in the third and sixth months of therapy compared with the basal levels, while the estradiol and HDL levels increased (p<0.001; for patch group HDL levels p=0.004). Remaining lipid profile parameters in the sixth month in the transdermal gel group decreased significantly compared with the basal levels. There was a significant decrease in the third month ALT level compared with the basal level, while AST levels didnt change throughout the study period in both groups (p=0.006).Conclusion: Transdermal estradiol therapy in the form of patch or gel are similar in efficacy, however transdermal estradiol gel has a lower rate of local skin reaction, is easier to apply with more patient pleasure.
