ÖZET
Amaç: Fetal komplet üretral atrezi saptanan bir olgunun tartışılması.Çalışmanın Yapıldığı Yer: Ege Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum Ana Bilim Dalı, İzmir.Olgu Sunumu: Rutin obstetrik kontrolü amacıyla kliniğimize müracaat eden hasta 35 yaşında gravida 3 para 2 idi. Gebeliği son adet tarihine göre 13 haftalık idi. Hastanın özgeçmişinde tip І diabetes mellitus mevcuttu. Fetal sonografik incelemede, fetusta megamesane ve bilateral pelvik kaliektazi yanında oligohidramnios saptandı. Yirminci gebelik haftasında yapılan kordosentez sonucu alınan fetal kan örneğinde yapılan kromozom analizi normal olarak bulundu. Bu bulgular sonucunda fetusda yaşam ile bağdaşmayan alt üriner sistem obstruksiyonu varlığı düşünülerek gebeliğin sonlandırılmasına karar verildi. Doğum indüksiyonu ile kardiak aktivitesi olmayan 650 gr. kız fetus doğurtuldu. Fetusun patolojik incelemesinde komplet üretral atrezi, megamesane, megaüreterler, bilateral hidronefroz yanında tek arter içeren göbek kordonu saptandı.Sonuç: Fetal alt üriner sistem obstruksiyonu olan olgularda erken tanı konması ayrıntılı sonografik inceleme ile olasıdır.
ABSTRACT
Objective: To report a case of complete urethral atresia in a fetus.Institution: Ege University Faculty of Medicine Department of Obstetrics and Gynecology, İzmir.Case Report: A 35-year-old woman gravida 3, para 2, with type І diabetes mellitus admitted to our clinic at the 13th gestational week. Fetal sonographic examination demonstrated fetal megavesica and bilateral renal pelvic caliectasia and oligohydramnios. The chromosomal analysis of fetal cord blood sample done at 20th week of gestation obtained by cordocentesis revealed normal karyotype. Termination of pregnancy was decided because of the presence of lower urinary tract obstruction incompatible with life. The patient delivered a 650 g female fetus without cardiac activity. Fetal pathological examination revealed complete urethral atresia, megavesica, megaureters, bilateral hydronephrosis and single umblical artery.Results: In cases with lower urinary tract obstruction, early diagnosis is possible with detailed sonographic examinaton of the fetus.
