Cyproterone Acetate, Spironolactone And Bromocriptine Treatment In Patients With Hirsutismus

ÖZET
Çalışma Ankara Hastanesi Endokrinoloji ve metabolizma hastalıkları bölümüne hirsutismus şikayeti ile başvuran 55 hasta üzerinde yapıldı. Hastalarda bazal, tedavinin 3. ayında ve 6. ayında hormon, biokimya ve klinik olarak kıl karakterindeki değişimler gözlendi. Hastalar androjen hormonların sekresyon özelliğine göre 3 gruba ayrılarak; hiperprolaktinemisi olan 8 hastada bromokriptin 5 mg/gün, serbest testosteronu yüksek 26 hastada cyproterone acetat 2 mg/gün, ethinylestrodiol 0.035 mg/gün (DianeR 35tb), serbest testosteronu ve DHEA-S'ı birlikte yüksek bulunan 13 hastada DianeR 35 tb'e ilaveten Spironolakton 200 mg/gün şeklinde uygulandı. Hirsutismusu olmayan 11 normal kadın kontrol grubuna alındı. Hiperprolaktinemili hastalarda tedavi ile prolaktin düşük (p<0.001), serbest testosteron ise anlamlı olarak düşük (p>0.1) değildi. Serbest testosteronu yüksek olan hastalarda tedavi ile prolaktin (p<0.05), serbest testosterone (p<0.001) ve DHEA-S (p<0.01) olarak anlamlı olarak düşük bulundu. Serbest testosteronu ve DHEA-S birlikte yüksek bulunan hastalarda tedavi ile serbest testosteron (p<0.01), DHEA-S (p<0.05) anlamlı olarak düşük bulundu.
ABSTRACT
This study was performed on 55 patients with hirsutismus who applied to Ankara Hospital Endocrinology and Metabolism clinic. The changes in the body hair character, hormone levels, blood chemistry were observed in the 3rd ve 6th months of basal treatment. The patients were divided into three groups according to the secretion of androgenes hormones 8 patients who have hiperprolactinemia were given bromocriptine 5 mg/day, 26 patients whose free testosterone level were elevated were given cyproterone acetate 2 mg/day, and ethynylestradiol 0.035 mg/day (DianeR 35 tb), 13 patients whose free testosterone and DHEA-S were both elevated were given DianeR tab plus spironolactone 200 mg/day. Eleven normal females who haven't got hirsutismus were chosen as the control group. The prolactine level after the treatment was low in patients with hyperprolactinemia (p<0.001) but the decrease in the free testosterone level was not statistically significant (p>0.1). The levels of prolactine (p<0.05), free testosterone (p<0.01) and DHEA-S (p<0.05) were low in patients with high free testosterone level after the treatment and these results were statistically significant. Free testosterone (p<0.01) and DHEA-S levels (p<0.05) were statistically significantly low after the treatment in patients whose free testosterone and DHEA-S were both high.
