Does Single ?-hCG Assay Predict The Pregnancy Outcome In ICSI Cycles?

ÖZET
Amaç: ICSI sikluslarında, gebelik sonucunu öngörmede embriyo transferi sonrası 12. günde tek bir ?-hCG ölçümünün klinik değerini saptamak.Gereç ve Yöntemler: Embriyo transferi sonrası 12. günde, serum ?-hCG konsantrasyonunun 10 IU/L ve üzerinde olduğu, gebelikle sonuçlanan 94 ICSI siklusunu inceledik. Serum ?-hCG konsantrasyonu, chemiluminescent enzyme immunoassay yöntemi ile ölçüldü. Gebelikler, viable (20. haftaya ulaşan gebelik) veya nonviabl ( biyokimyasal gebelik, abortus ve ektopik gebelik) olarak sınıflandırıldı.Bulgular: Median ?-hCG konsantrasyonu, viable gebeliklerde 138 IU/L, nonviabl gebeliklerde 55 IU/L idi (p=0,001). Median ?-hCG konsantrasyonu, tekil gebeliklerde 121 IU/L, çoğul gebeliklerde 265 IU/L idi (p=0,005). Değişik infertilite etyolojilerindeki median ?-hCG konsantrasyonları arasında istatistiksel olarak anlamlı fark saptanmadı.Sonuç: Viable gebeliği öngörmede, embriyo transferi sonrası 12. gündeki tek serum ?-hCG değeri için cut -off değeri saptayamadık. Bu nedenle IVF protokolümüzde, 14. günde ?-hCG ölçümünü tekrarlamaya devam etmenin yararlı olacağı kanısına vardık
ABSTRACT
Objective: Our aim was to assess the clinical value of a single 12 day after embryo transfer ?-hCG assay to predict the outcome of pregnancy in ICSI protocols.Material and Methods: We studied 94 ICSI cycles resulting in pregnancy defined as a serum ?-hCG concentration of ? 10 IU/L on day 12 following embryo transfer . Serum ?-hCG concentrations measured by chemiluminescent enzyme immunoassay. Pregnancies were classified as viable (live fetus at ? 20 weeks gestation) or nonviable ( biochemical pregnancy, miscarriage and ectopic pregnancy).Results: The median ?-hCG concentration was 138 IU/L in viable pregnancies and 55 IU/L in nonviable pregnancies (p=0,001).The median ?-hCG concentration was 121 IU/L in singleton pregnancies and 265 IU/L in multiple pregnancies (p=0,005). There was no statistical difference between median ?-hCG concentrations of different infertility etiologies.Conclusion: We could not determine a cut off level of single serum ?-hCG concentration after 12 day embryo transfer for predicting viable pregnancy. Therefore, we will continue to repeat ?-hCG assay on 14 day after ET in our IVF protocol.
