Efficiacy Of Postpartum Oral Glucose Tolerance Test To Evaluate The Glucose Metabolism In Women That Delivered Macrosomic Fetuses

ÖZET
Amaç: Makrozomik doğum yapmış kadınlarda erken postpartum dönemde yapılan 100 gram oral glukoz tolerans testinin glukoz metabolizması bozukluğunu belirlemedeki etkinliğini saptamak. Gereçler ve Yöntemler: Kliniğimizde Ocak-Temmuz 2000 tarihleri arasında, makrozomik doğum yapan kadınlar çalışma grubunu oluştururken (n:55), normal ağırlıkta bebek doğuran kadınlar kontrol grubunu (n:40) oluşturdu. Olgulara gebelikleri sırasında glukoz tolerans testi uygulanmamıştı. Gruplara erken postpartum dönemde 100 gram OGTT ve HbA1C ölçümü yapıldı. Gruplar; makrozomik doğum öyküsü, parite, BMI, doğum şekli, postpartum 100 gram OGTT sonuçları ve HbA1C değerleri açısından karşılaştırıldı. Bulgular: Yaş, BMI, doğumda gebelik haftası ve parite açısından gruplar arasında istatistiksel olarak anlamlı bir fark saptanmadı. Çalışma grubunda makrozomik doğum öyküsü olguların %35.1inde saptanırken, kontrol grubunda bu oran %11.5 olarak belirlendi (p<0.05). Postpartum OGTT çalışma grubunda %25.5 oranında pozitif olarak belirlendi. Kontrol grubunda OGTT pozitifliği ancak grubun %5inde belirlendi (p<0.05). Sonuç: Makrozomik doğum yapan kadınlara, gebelik sırasında tanı konulamamış glikoz intoleransının belirlenmesi ve buna bağlı olarak gelecekte gelişebilecek tip 2 diabetin erken tanısı ve dolayısıyla komplikasyonlarının kontrolü için doğum sonrası erken dönemde OGTT uygulanmasının yararlı olacağı kanısındayız.
ABSTRACT
Objective: To detect the efficacy of OGTT to detect the tendency of impaired glucose metabolism during the early postpartum period in women that delivered macrosomic fetuses. Material and Methods: The cases that delivered macrosomic fetuses in our clinic formed the study group and the cases that delivered fetuses with normal birth weight served as the control group between January and July 2000. GTT was not performed to the cases during the pregnancy. OGTT and HbA1C measurement were performed to the groups during early postpartum period. Groups were compared according to the terms of history of having macrosomic fetuses at previous pregnancies, parity, BMI, type of delivery, results of oral glucose tolerance test and HbA1c. Results: There is no significant differences between the groups in terms of age, BMI, parity, gestational week at the time of delivery. History of macrosomic delivery at previous pregnancy was detected 35,1% of cases in study group and 11,5% of control group (p<0.05). Postpartum OGTT was positive 25,5% of cases in study group and 5% of control group (p<0.05). Conclusion: We concluded that, performing of OGTT during the early postpartum period in women that delivered macrosomic fetuses, enabled the diagnosis of glucose intolerance which was not detected during pregnancy, that could lead to type -2 diabetes and helped to control complications due to a possible diabetes.
