Maternal And Perinatal Outcomes After Uterine Rupture In Pregnancy

ÖZET
Amaç: Uterus rüptürü gelişen hastalarda risk faktörlerini belirlemek, maternal ve perinatal sonuçları değerlendirmektir. Gereç ve Yöntemler: 1999-2003 tarihleri arasında kliniğimizde tespit edilen gebelikte görülen 30 uterus rüptürü olgusu retrospektif olarak değerlendirildi. Bulgular: Rüptür görülme oranı %0.29 idi. Olguların %46.7sinde komplet rüptür mevcuttu. Hastaların %80i en az 1 uterin insizyona sahip iken %20sinde uterin insizyon yoktu. Olguların %46.7sine preoperatif, %53.3üne intraoperatif tanı konuldu. Komplet rüptür gelişen olgularda klinik olarak rüptür belirtileri inkomplet olanlara göre daha çok tespit edildi. (p< 0.001) ve hastanede kalış süreleri daha uzundu (p< 0.05). Rüptüre bağlı intrauterin ölü fetus (İUÖF) yalnızca komplet rüptürlü hastalarda gelişirken (p< 0.001) canlı doğurtulan fetusların 5. dk. Apgar skorları da bu grupta daha düşük idi (p< 0.05). Uterin cerrahi hikayesi olmayan 6 hastanın tamamı grand multipar idi. Tüm olgularda primer tamir uygulandı. Maternal mortalite görülmedi. Perinatal mortalite oranı %33.3 idi. Sonuç: Rüptürün klasik belirtileri her zaman görülmeyebilir. Hızlı yapılacak operatif müdahale, bebek ve anne açısından hayat kurtarıcı olabilir. Risk grubundaki gebelerin yakın takip altında bulundurulması ve acil şartlara hazırlıklı olunması gerekmektedir. Ancak, rüptür yalnızca uterin insizyonu bulunan gebelerde değil skarsız uteruslarda da gelişebilir. Ülkemizdeki ileri yaştaki gebeliklerin sıklığı da göz önüne alınarak grand multipar olguların rüptür gelişimi açısından çok dikkat edilmesi gereken grubu oluşturduğu hatırda tutulmalıdır.
ABSTRACT
Objective: To detect risk factors for uterine rupture and evaluate maternal and perinatal results in patients with uterine rupture. Material and Methods: Thirty patients diagnosed with uterine rupture in our clinic between 1999-2003, were evaluated retrospectively. Results: The overall rate of uterine rupture was 0.29%, of those patients, 46.7% had complete rupture. While 80% of patients had one or more uterine incision, 20% of cases didnt have any uterine incision. The diagnosis could made preoperatively for 53.3% of patients, and during cesarean for 46.7% of patients. The complete rupture patients had more evident clinical signs than incomplete rupture cases (p< 0.001), and longer of stay in hospital (p< 0.05). In utero mort fetus only occurred in patients with complete rupture (p< 0.001), and 5-minute Apgar scores in live births were lower in this group (p< 0.05). All of the six patients who have unscarred uterus were grand multipare. The primary repair of rupture was done in all patients. There were no maternal deaths. The rate of perinatal mortality was 33.3%. Conclusion: The classical signs of uterine rupture might not be seen everytime. Prompt surgical intervention can save life of fetus and mother. The pregnant women who have risk factors must be closely followed and be prepared to emergency condition. But, uterine rupture can be occur not only in patients who had incision of uterus but also unscarred uterus. As grand multiparity is one of the important obstetrics problems in our country, one should be kept in mind the high risk of uterine rupture in grand multipar patients.
