Good Perinatal Outcome After Serial Amniocentesis And Strict Glycemic Control In A Twin Pregnancy Complicated By Gestational Diabetes Mellitus: Case Report

ÖZET
Gestasyonel diabetes mellitus (GDM) ile komplike gebeliklerde fetal mortalite, morbidite ve obstetrik girişim riski yükselir. İkiz gebeliklerde bazı plasental anomaliler saptanmışsa da monokoryonik plasenta ile birlikte ikiz-ikiz transfüzyon sendromu olmadan polihidramnios-oligohidramnios görülmesi çok nadirdir. Ayrıca GDM?li hastalarda polihidramnios beklenebilen bir problemdir ve sıkı glikoz kontrolü ile düzeltilebilir. Burada muhtemel plasental vasküler patolojiye bağlı polihidramnios-oligohidramnios izlenen GDM?li diamniotik-dikoryonik ikiz gebe sunulmuştur. Böyle hastaların yönetiminde seri amniosentez ve normogliseminin sağlanmasıyla gestasyonel yaş uzatılıp olumlu perinatal sonuçlar elde edilebilir.
ABSTRACT
Fetal mortality, morbidity and the need for obstetric interventions are increased in pregnancies that were complicated by gestational diabetes mellitus (GDM). Although some placental abnormalities have been observed in twin pregnancies, polyhydramnios-oligohydramnios is extremely rare otherwise there is monochorionic placentation with twin-twin transfusion syndrome. Also, GDM can cause polyhdramnios that might be overcome by tight glycemic control. Here we presented a woman with diamniotic-dichorionic twin pregnancy who had polyhydramnios-oligohydramnios that was probably caused by placental vascular pathology. Serial amniocentesis and tight glucose control may prolong pregnancy interval and result in good perinatal outcome in such patients.
