PLASMA ? -ATRIAL NATRIURETIC PEPTIDE LEVELS IN HYPERTENSIVE PREGNANCIES

ÖZET
Amaç: Hipertansif gebeliklerde plazma ? -Atrial Natriüretik Peptid (? -ANP) düzeylerinin tespiti ve klinik çalışmalardaki önemi. Çalışmanın Yapıldığı Yer: Gazi Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum AD, Ankara. Materyel ve Metod: Otuzbir hipertansif ve 24 normal gebede RIA yöntemi ile plazma ? -ANP düzeyleri ölçüldü. Bulgular: Plazma ? -ANP düzeyleri, hipertansif grupta 22.4 ± 17.1 (A.Ort. ± S.Sapma) pg/ml ve kontrol grubunda 15.1 ± 8.0 pg/ml olarak saptandı. Hipertansif grupta ? -ANP düzeylerinin yüksek olduğu görüldü (p< 0.05). hipertansif alt gruplar incelendiğinde ? -ANP düzeyleri hafif preeklampsi grubunda 15.1 ± 6.9 pg/ml, kronik hipertansif grupta 15.6 ± 5.3 pg/ml ve şiddetli preeklampsi grubunda 32.9 ± 22.2 pg/ml tespit edildi. ?-ANP düzeylerinin şiddetli preeklampsi grubunda belirgin yüksek olduğu bulundu (p< 0.05). Sonuç: Elde ettiğimiz plazma ? -ANP değerlerinin şiddetli preeklampsi ve eklampsi klinik tablosu ile yakın korelasyonu, ? -ANP'nin preeklampsi etyopatogenezindeki değerini, hastalığın tanı takip ile tedavisindeki geçerliliğini göstermektedir.
ABSTRACT
Objective: To determine the plasma ? -Atrial Natriuretic Peptide (?-ANP) levels in hypertensive pregnancies and its importance in clinical studies. Institution: Department of Gynecology and Obstetrics, Gazi University Medical Faculty. Material and Methods: In our study, plasma levels of ? -ANP has been assayed in 31 hypertensive and 24 normal pregnant women by an extraction procedure included in Radio Immuno Assay (RIA). Findings: Plasma ? -ANP values for main groups in our study were as follows; 22.4 ± 17.1 (mean ± SD) pg/ml in the hypertensive group and 15.1 ± 8.0 pg/ml in the control group. The values were found to be statistically high in the hypertensive group (p< 0.05). When the hypertensive subgroups were investigated, the following results were obtained: ? -ANP values were 15.1 ± 6.9 pg/ml in the mild preeclampsia group, 15.6 ± 5.3 pg/ml in the chronic hypertensive group and 32.9±22.2 pg/ml in the group with severe preeclampsia. The values were found to be statistically high in the group with severe preeclampsia (p< 0.05). Results: The high correlation of plasma ? -ANP values, which we found in this study, with clinical states of preeclampsia and eclampsia showed the value of ? -ANP in the etiopathogenesis of preeclampsia; and its validity in the diagnosis and management of the disease.
