ÖZET
Amaç: Primer veya sekonder infertilite tanısı ile başvuran ve uterin septum saptanan olgularda histeroskopik septum rezeksiyonu sonrası konsepsiyon ve gebelik hızlarının retrospektif olarak saptanması. Çalışmanın Yapıldığı Yer: Hacettepe Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum AD, Ankara. Materyel ve Metod: Eylül 1993-Aralık 1994 tarihleri arasında primer veya sekonder infertilite nedeni ile kliniğimize başvuran 46 hastada HSG'de uterin septum tespit edildi. 21 hasta primer infertilite; 25 hasta ise daha önce düşük ve/veya immatür doğum öyküleri nedeni ile sekonder infertilite grubunda idi. Tüm olgulara infertilite'ye yönelik ayrıntılı bir çalışma sonrası histeroskopik yolla septum rezeksiyonu uygulandı. Bütün hastalara histeroskopi ile eş zamanlı laparaskopi uygulandı. Bulgular: Septumların 18'i uterin korpus içinde idi. 17 olguda istmus'a; 11 olguda da servikal kanala uzanan septum görüldü. Ortalama operasyon süresi 25.5 ± 7.0 dk. İdi. Tahmini kan kaybı 5-50 cc olarak saptandı. 3 vakada işlem sırasında uterus perforasyonu oldu. Uterus perforasyonu olan bir olgu dışında, tüm olgularda postoperatif kontrol HSG'de septumun ortadan kalktığı gözlendi. Histeroskopik rezeksiyon sonrası 21 olguda (%45.6) konsepsiyon sağlandı. Bu 21 olgunun 18'inde (%39.1) gebelik terme kadar ulaştı. Primer ve sekonder infertil kabul edilen grupta term gebelik hızı sırasıyla %33.3 ve %56 olarak bulundu. Sonuç: Histeroskopik septum rezeksiyonu gerek primer, gerekse sekonder infertil olgularda fertiliteyi olumlu bir ölçüde artırmaktadır. Bu yüzden infertilite veya tekrarlayan gebelik kayıpları saptanan olgularda septum varlığı araştırılmalıdır. Bu tür olgularda operasyon süresinin ve hastanede kalış süresinin kısalığı, ayrıca daha sonraki gebeliklere vajinal doğum şansı tanıması açısından histeroskopik yaklaşım tercih edilmelidir.
ABSTRACT
Objective: To determine the conception and term pregnancy rates following the hysteroscopic resection of the primary or secondary infertile cases in which the etiology was found to be the uterine septum. Institution: Department of Obstetrics and Gynecology, Hacettepe University Faculty of Medicine. Material and Methods: In 46 patients admitted to our department, from September 1993 to December 1994, with primary or secondary infertility (previous abortions, premature deliveries), uterine septum was detected via HSG. Primary and secondary infertility categories consisted of 21 and 25 patients, respectively. Following the exclusion of all other factors leading to the infertility, hysteroscopic septum resection and concomittantly laparascopy were performed in all cases. Results: The mean operating time was 25.5 ±7.0 mn, and the estimated intraoperative blood loss ranged from 5 to 50 cc. There were 3 uterine perforations as an intraoperative complication. In all cases, apart from one case in which the uterus was perforated, postoperative HSG revealed no remnant tissue. 21 patients have conceived (45.6%) following the resection. Of these 21 cases, term pregnancy was achieved in 18 patients (39.1%). In primary and secondary infertile groups, this figure was found to be 33.3% and 56%, respectively. Conclusion: Hysteroscopic septum resection promotes the fertility either primary or secondary. Thus, any case with a history of recurrent pregnancy was stages and infertility necessitates the investigation for the presence of the uterine septum. Hysteroscopic surgical approaches should be preferred in regard to the short duration of the operation and the hospital stay as well as the increased chance for the vaginal delivery in the next pregnancies following the operation.
