Intact Versus Crushed Tablets Of Intravaginal Misoprostol For Cervical Ripening: Preliminary Results Of A Prospective Randomized Trial

ÖZET
Amaç: Vaginal misoprostolün intakt ya da ezilmiş olarak uygulanmasının servikal yumuşama ve doğum indüksiyonu üzerine olan etkilerinin karşılaştırılmasıMateryel ve Method: Bishop skoru <5 ve gestasyonel haftası >35 hafta olan 61 gebe çalışmaya dahil edildi. Olgular randomize olarak iki gruba ayrıldı: (Grup 1) 50 mcg misoprostol intakt tablet (n=29), (Grup 2) 50 mcg misoprostol ezilmiş tablet (n=32) intravaginal olarak uygulandı. Etkinlik ve yan etki açısından iki grup karşılaştırıldı.Sonuçlar: Grup 1de vaginal doğum 19.4 ± 10.3 saatte, grup 2de ise 16.9 ± 11.8 saatte gerçekleşti (p<0.05). 24 saat içinde vaginal doğum yapma bakımından aradaki fark anlamsızdı (Grup 1de %69, grup 2de ise %75 olgu, P>0.05). Yan etkiler bakımından iki grup arasında fark yoktu (P>0.05).Tartışma: Çalışmadaki verilere göre iki grup arasında etkinlik ve yan etkiler açısından anlamlı fark olmamakla birlikte, ezilmiş tablet uygulanımı daha erken doğumla sonuçlanması sebebiyle, doğumun bir an önce gerçekleştirilmesi gereken uygun olgularda tercih edilebilir.
ABSTRACT
Objective: To compare the effects of intact or crushed intravaginal misoprostol tablets on cervical ripening and labor induction.Materials and Methods: Sixty-one pregnant women with gestational age >35 weeks and Bishops score <5 were included in the study. Patients were randomized into 2 groups: Group 1 (n=29)received 50mcg intact misoprostol tablet; and group 2 (n=32) received 50mcg crushed misoprostol tablets intravaginally. Two groups were compared with respect to effectiveness and side effects.Results: The mean (±SD) delivery time was 19.4±10,3 hours in group 1, and 16.9±11.8 hours in group 2 (p<0,05). There was not any difference between the groups with respect to delivery within 24 hours (69% in group 1, and 75% in group 2, p>0.05). Side effects were similar in both groups (p>0.05).Discussion: There was not any significant difference between the groups with respect to efectiveness and side effects. However, delivery occured earlier in crushed tablet group, so it may be considered in suitable clinical conditions where rapid delivery is desired.
