Laparoscopic Tubal Sterilization with Harmonic Scalpel: A Review of 33 Cases

ÖZET
Amaç: Harmonik makas (HM) ile laparoskopik tubal ligasyon işleminin cerrahi sonuçları, intraoperatif-postoperatif komplikasyonları ve erken dönem başarısızlık oranlarını değerlendirmek. Gereç ve Yöntemler: Şubat 2008-Ekim 2009 tarihleri arasında kalıcı sterilizasyon yöntemi ile korunmak için başvuran 33 kadına aynı cerrahi ekip tarafından harmonik makas ile laparoskopik tubal ligasyon işlemi uygulanmıştır. Gebe olan 12 olguya sterilizasyon ile eş zamanlı cerrahi abortus işlemi uygulanmıştır. Hastaların demografik özellikleri, intraoperatif, postoperatif komplikasyonlar, operasyon ve hastanede kalış süreleri değerlendirilmiştir. Bulgular: Hasta yaşlarının ortalama değeri 36 ± 3.1 (30-41 arası), ortalama parite 3 ± 0.9 (2-5 arası) idi. Olguların 12 (%36.3ü)si gebe ve ortalama gebelik yaş 41 ± 3 (35-50 arası) gün idi. Ortalama operasyon süresi gebe olmayan olgular için 10.6 ± 2.76 (8-20 arası) dakika, gebe olgularda ise 10.9 ± 1.50 (9-14 arası) dakika idi. Olguların hiçbirinde intraoperatif kanama ya da komşu organ yaralanması izlenmedi. Çalışmada yer alan hastaların tümü operasyon günü taburcu edildi ve hastanede yattıkları süre içerisinde ek analjezik gereksinimi olan hasta olmadı. Ortalama 19.8 ± 7.1 (10-29 arası) ay olan izlem süresi içerisinde gebelik ya da ektopik gebelik olgusu izlenmedi. Sonuç: Harmonik makas ile laparoskopik tubal ligasyon operasyonu, basit ve güvenle uygulanabilen bir sterilizasyon yöntemdir. Gebelik varlığı cerrahi tekniğin uygulanabilirliğini etkilemediği için cerrahi abortus işlemi ile eş zamanlı olarak da uygulanabilmektedir.
ABSTRACT
Objective: The purpose of this study is to evaluate the surgical results, intraoperative and postoperative complications, and early failure rate of laparoscopic tubal sterilization with harmonic scalpel (HS). Material and Methods: Laparoscopic tubal sterilization with harmonic scalpel was performed by the same surgical team for thirty three woman who want to have permanent sterilization between February 2008 and October 2009. In addition, twelve of those pregnant women were also had undergone surgical abortion at the same time. The patients demographic characteristics, complications, duration of the procedure and hospital stay were recorded. Results: The median age of the patients was 36 ± 3.1 (between 30-41) years, and the median value of the parity was 3 ± 0.9 (between 2-5). Twelve women (36.3%) were pregnant and the mean gestational age for the pregnant women was 41 ± 3 days (between 35-50 days). The mean operating times were 10.6 ± 2.76 (between 8-20) minutes and 10.9 ± 1.50 (between 9-14) minutes for non-pregnant and pregnant group, respectiveley. Intraoperative bleeding and adjacent tissue injury were not observed. Additional analgesic requirement was not observed during the patients hospital stay. All the patients were discharged from the hospital on the same day that they were operated. The mean follow up period was 19.8 ± 7.1 (between 10-29) months, and no pregnancies (intrauterine and extrauterine) was reported during this period. Conclusion: Laparoscopic tubal sterilization with harmonic scalpel is a simple and safe sterilization method that is satisfactory for both patients and gynecologists. Because of its safetyness for pregnant women, it can be performed concomitantly with surgical abortus.
