Lupus Anticoagulant Antibodies In Cases With Recurrent Pregnancy Loss

ÖZET
Amaç: Tekrarlayan gebelik kayıplarında immunolojik faktörlerden Lupus Antikoagülan Antikor pozitifliğinin araştırılması ve bu araştırmada nasıl bir yöntem uygulanacağının belirlenmesi amaçlandı. Çalışmanın Yapıldığı Yer: Zeynep Kamil Hastanesi, İstanbul. Materyal ve Metod: Tekrarlayan gebelik kayıpları olan 40 olgu ve kontrol grubu olarak bir ya da iki doğumu olan, hiç düşüğü olmayan ve son trimesterde başvuran 20 gebede Lupus Antikoagülan Antikoru araştırıldı. Lupus Antikoagülan Antikorunu tespit etmek için tarama testi olarak APTT ve spesifik test olarak da PNP kullanıldı. Her iki grubun sonuçları karşılaştırıldı. Bulgular: Tekrarlayan gebelik kapıları olan 40 olgunun üçünde APTT değerleri uzamış olarak bulunurken (%7.5), kontrol grubundaki hiç bir olguda APTT değerlerinde uzama tespit edilmemiştir (%0.0). Uzamış APTT değerlerine sahip olgulara PNP-APTT ve Salin APTT prosedürleri uygulandığında APTT değerlerinde belirgin kısalma olduğu, normal APTT'li tekrarlayan gebelik kayıpları olan olgularda ve kontrol grubunda PNP-APTT testini uyguladığımızda sonuçlarda hiç bir değişiklik olmadığı gözlenmiştir. Sonuç: Sebebi açıklanamayan tekrarlayan gebelik kayıpları olan olgularda immunolojik faktörlerden Lupus Antikoagülan Antikor araştırılması başarı şansını yükseltecektir. Pahalı ve zaman alıcı bir test olan PNP'nin spesifik bir test olarak bilinmesi ve sadece uzamış APTT'li olgulara uygulanması, APTT'nin ise bir tarama testi olarak tüm açıklanmamış tekrarlayan gebelik kayıpları olgularına uygulanması yerinde bir karar olacaktır.
ABSTRACT
Objective: Detection of Lupus Anticoagulant Antibodies as an immunologic factor in cases with an history of recurrent pregnancy loss and determination of the priority of the laboratory methods in this investigation. Institution: Zeynep Kamil Maternity Hospital, İstanbul Material and Methods: Lupus Anticoagulant Antibody was investigated in the sera of 40 cases with and history of recurrent pregnancy loss and as a control group in 20 third trimester pregnancies with one or two deliveries but no abortions. APTT test was used for screening and PNP as a spesific test. Results of both groups were compared. Results: As the APTT results were found to be prolonged in 3 of the 40 cases with an history of recurrent pregnancy loss (7.5%). APTT results of all cases in the control group were found to be in the normal range (0.0%). PNP-APTT and Saline APPT procedures were determined in cases with prolonged APTT results and marked changes were observed as no differences in the results were observed in recurrent pregnancy loss cases with normal APTT results and in the control group when the PNP-APTT test was used. Conclusion: Investigation of Lupus Anticoagulant Antibody in unexplained cases of recurrent pregnancy loss will optimize the success rate. It is a good policy to use the APTT test as a screening method in all cases of unexplained recurrent pregnancy loss and reserve the expensive and time-consuming PNP test as a spesific test for cases with prolonged APTT results.
