ÖZET
1989 yılı son 6 ayı içinde idrar kaçırma şikayeti ile kliniğimize başvuran 23 hastanın basit sistometrik test ve metal tesbit zincirli sistoüretrografi yardımıyla ürodinamik ve radyolojik yönden tetkikleri yapıldı. Bu tetkikler sayesinde stres inkontinensin detrüsör instabilitesi ile ayırıcı tanısı yapılarak uygulanacak ameliyat yöntemi seçildi. Vakalarımızın 3'ünde detrüsör instabilitesi saptanarak medikal tedavi verildi. 2'sinde mesane tümörü ve divertikülü saptanarak üroloji kliniğine sevk edildi. 18 vakaya da sistoüretrografik tiplendirme yapılarak uygun cerrahi tedavi uygulandı. Hastaların ameliyat sonrası başarı değerlendirilmeleri de sistoüretrografik olarak yapılıp, genel başarı oranı %78 bulundu. Bu iki yöntemin stres inkontinensli vakaların pre- ve postoperatif değerlendirilmesinde önemli ve değerli olduğu, bu nedenle stres inkontinens şikayeti ile başvuran tüm hastalara rutin olarak uygulanması gerektiği sonucuna varıldı.
ABSTRACT
During the last six months of 1989, 23 patients with the complaints of urinary incontinence were evaluated both radiologically and urodynamically by metallic bead-chain cystouretrography and simple cystometry. By the aid of these tests differential diagnosis between stres urinary incontinence and detrusor instability and choise of appropriate operation were performed. Theree of our cases were diagnosed to have detrusor instability and were sent to a urology clinic. 18 cases were classified cystouretrographically and appropriate surgical treatment applied. Postoperative surgical failure was evaluated cystouretrographically and our general surgical success rate was 78%. It was concluded that two methods were valuable in evaluation of stres urinary incontinence cases.
