ÖZET
Amaç: Minör jinekolojik cerrahi geçirecek olan hastalarda çift kör yöntemi ile preemptif olarak uygulanan diflunizalin etki ve yan etkilerini araştırmayı amaçladık. Çalışmanın Yapıldığı Yer: Gülhane Askeri Tıp Akademisi, Anesteziyoloji ve Reanimasyon Anabilim Dalı, Ankara. Materyal ve Metod: ASA I ve II, 18-35 yaşları arası minör jinekolojik operasyon geçirecek 40 olgu çalışıldı. Premedikasyon uygulanmayan olgular 3 gruba ayrıldı (n=20/grup). I. ve II. grup olgulara çift kör yöntemi ile preemptif olarak 500 er mg'lık iki tablet halinde oral diflunizal veya plasebo yaklaşık 20-30 ml su ile verildi. Çalışmaya katılmak istemeyen 20 hasta ile de III. grup oluşturuldu. Olgulara aynı anestezi protokolü uygulandı. Operasyon sonrası 5. ve 60. dakikalarda vizüel analog skala (VAS) ile olguların ağrıları değerlendirildi. Olguların klinikte 6 saat ve daha sonra evlerinde 12-18 saat boyunca ağrıları izlendi. Sonuçlar Student t testi ve ANOVA ile değerlendirildiler, p<0.05 anlamlı kabul edildi. Bulgular: Demografik veriler açısından gruplar arası anlamlı bir farklılık yoktu. Çalışma süresince VAS skoru diflunizal grubunda diğer gruplardan anlamlı ölçüde düşük bulundu. Diflunizal grubunda ek analjezik gereksinimi olmazken plasebo grubunda 12, kontrol grubunda da 17 olguda ek analjezik kullanıldı. Çalışma süresince diflunizal ve plasebo gruplarında yan etkiyle karşılaşılmadı. Sonuç: Sonuç olarak preemptif diflunizalin minör jinekolojik operasyon geçiren olgularda etkin ve uzun süreli postoperatif analjezi sağladığı ve uygulanan dozla yan etki oluşturmadığı kanısına vardık.
ABSTRACT
Objective: We aimed to investigate the preemptive analgesic and the adverse effects of diflunisal, in a double-blind manner, on patients undergoing minor gynecological surgery. Institution: Gülhane Military Medical Acedemy, Department of Anesthesiology and Reanimation, ANKARA Material and Methods: 40 subjects, ASA I and II, ranging in age from 18 to 35 years were included in this study. No premedication were given. Subjects were allocated in three groups (n= 20/group). In a double-blind manner preemptively two tablets of diflunizal or placebo, 500 mg, with 20-30 ml of water were given to the group I and II subjects. Group III was organised with subjects who did not want to participate in the study. The same anesthetic procedure were administered to all subjects. Their pain scores were evaluated using visual analogue scala (VAS) at 5th and 60th minutes after the operations. Pain reduction was monitored for 6 hours at ward and for 12-18 hours at home. Results were analysed by using Student's t-test and ANOVA, and p<0.05 was considered as significant. Results: There was no significant difference between groups according to the demografic features. Along the study, VAS scores in the diflunisal group were lower than the other groups. No additional analgesic was needed in diflunisal group while it was 12 in placebo group and 17 in control group. No adverse effect was seen in both diflunisal and placebo group. Conclusion: As a result, Diflunisal, as a preemptive analgesic, provided effective and long-term postoperatif analgesia on subjects undergoing minor gynecological surgery, and we did not see any advers effect with the dose we used.
