ÖZET
Amaç: Nörofibromatosisli iki gebe hastada rastladığımız obstetrik komplikasyonları sunmak. Çalışmanın Yapıldığı Yer: Başkent Üniversitesi Adana Uygulama ve Araştırma Merkezi Kadın Hastalıkları ve Doğum Kliniği. Materyel ve Metod: Nörofibromatosis ve gebeliği olan 2 hastanın herikisinde de hipertansiyon saptanması üzerine hospitalizasyonlarına karar verildi. Bulgular: 1. olguda preeklampsi tanısı konuldu ve tansiyon kontrol altına alındıktan sonra yapılan seri sonografilerde gelişme geriliği düşünüldü. Spontan kontraksiyonlar sonrası fetal distress tanısıyla sezaryene alındı. Eylemi başlamış olan 2. olguda yapılan sonografi de fetal kardiak aktivitenin olmadığı gözlendi. Spontan vajinal doğum sağlandı. Her iki vakada da postpartum komplikasyon olmadı. Sonuç: Gebelik nörofibromatosis ilişkisi konusunda tartışmalar mevcuttur. Hipertansiyon, intrauterin gelişme geriliği, ölü doğum, spontan abortus, preterm eylem gibi komplikasyonların arttığını bildiren çok sayıda olgu sunumu mevcut olmakla birlikte geniş serili tek çalışmada nörofibromatosisin obstetrik komplikasyonları arttırmadığı bildirilmiştir. 2 olgumuzda da gebeliklerin komplikasyonlarla seyretmiş olması bu hastaların gebeliklerinin tartışmalar ortadan kalkıncaya kadar dikkatli takip edilmesi gerekliliğini desteklemektedir.
ABSTRACT
Objective: To present obstetric complications in two pregnant patients with neurofibromatosis. Institution: Başkent University Adana Obstetrics And Gynecology Department. Material and Methods: Two pregnant patients with neurofibroma tosis were hospitalized with the diagnosis of hypertension. Findings: Preeclampsia was diagnosed in the first case and after the hypertension stabilized, intrauterine growth retardation was suspected during the serial sonogrophic examinations. After spontaneus contractions began the patient was delivered by cesarean section for fetal distress. The second hypertensive case was in labor and fetal cardiac activity was absent in sonography. She delivered vaginally. In both case there were not postpartum complications. Conclusion: There are controversies about the relationship between pregnancy and neurofibromatosis. There are a lot of case reports about inreased hypertension, intrauterine growth retardation, stillbirth, spontaneous abortion and preterm delivery rates in these patients. However in the largest series it is thought that neurofibromatosis does not increase obstetric complications. As we seen complications in our 2 cases we recommend follow up of these patients pregnancies must be done carefully until this contraversies resolves.
