Our Experience With Chorionic Villus Sampling: Invasive Procedures, Cytogenetic Analyses And Fetal Outcomes

ÖZET
Amaç: Koryon villus dokusu 1981'den beri genetik hastalıkların tanısında yaygın olarak kullanılmaktadır. Gebeliğin dokuzuncu haftasından terme kadar uygulanabilirlik ve hızlı sonuç verme gibi özellikleri olan bu yöntemin kliniğimiz rutinine koyulması ile ilgili deneyimlerimiz bu çalışmada sunulmaktadır. Materyel ve Metod: Deneysel TS-KVB ve TA-KVB serilerinden sonra diyagnostik serilere geçildi. Deneysel TS-KVB serisindeki 21 girişimde DP ile diyagnostik serilerde ikisi hariç tüm girişimlerde DP ve/veya HK ile sitogenetik analiz yapıldı. Bulgular: Öncelikle 93 TS-KVB'den oluşan deneysel seriye başlandı. Ardından altı TS-KVB girişiminden oluşan diyagnostik seri "tek iğne" TA-KVB girişiminden oluşan diyagnostik seri tamamlandı. "Tek iğne" TA-KVB'lerden dokuzu ölü fetuslarda uygulandı. Yüzkırk gebelikteki 142 "tek iğne" TA-KVB'de en büyük indikasyon grubunu "ileri anne yaşı" oluşturdu. Girişimlerdeki ortalama gebelik haftası 16.54 ± 5.38 hafta (en küçük 10, en büyük 39 hf.), girişimlerde başarı oranı %97.88 (139/142) idi. Bu grupta girişim sonrası toplam fetal kayıp oranı %5 (7/140) olarak belirlendi. Fetal kayıp açısından "düşük riskli" grupta ortaya çıkan "girişim sonrası toplam fetal kayıp" oranı ise %2.9 (2/68) idi. 135 gebelikte uygulanan 137 "tek iğne" TA-KVB ile elde edilen villuslardan 18'inde DP, 13'ünde HK ve diğerlerinde her iki teknik ile sitogenetik incelemeler yapıldı. Sitogenetik başarı oranları: DP'de %94.5, HK'nde %92.3 ve DP/HK'da ise %96.2 idi. Tüm serideki sitogenetik başarı oranı ise %95.5 idi. Tüm serideki sitogenetik başarı oranı ise %95.5 idi. Kromozomal anomali oranı tüm seride %8.5 (11/135), "ileri anne yaşı" grubunda %7.31 (3/41), "patolojik ultrason" grubunda %14.7 (5/34) olarak saptandı. Yanlış pozitif plasenta ile sınırlı mozaisizm oranı %1.4 idi. Maternal hücre kontaminasyonuna rastlanmadı. Tartışma ve Sonuç: Bulgular literatür ile tartışıldı ve KVB'nin gerek invazif girişim tekniği olarak gerekse sitogenetik değerlendirme açısından kliniğimize transferinin tamamlandığı kararına varıldı.
ABSTRACT
Introduction: Chorionic villus sampling procedures have been widely used since 1981 for the prenatal diagnosis of genetic diseases. Our experiences in establishing in our clinic this safe and effective procedure are presented. Material and Methods: Our diagnostic series began after experimental transcervical and transabdominal chorionic villus sampling (TC-CVS and TA-CVS) series. In 21 experimental TC-CVS, the cytogenetic results were obtained by direct method only, however, in diagnostic series, all except two cases were analyzed cytogenetically both by direct and/or culture methods. Results: The experimental series included 93 TC-CVS and nine "single needle" TA-CVS procedures. The diagnostic series has been completed following six-TC-CVS, and four "double needle" and 151 "single needle" TA-CVS procedures. Of the TA-CVS, nine were performed to patients with dead fetuses. The main indication group was "advanced maternal age" in 142 "single needle" TA-CVS procedures performed to 140 patients. The mean gestational week in TA-CVS interventions was 16.54 ± 5.38 (min:10, max:36). The success rate of the procedure was 97.88% (139/142). The fetal loss rate was determined as 5% (7/140). In the group of "low risk" pregnancies for the fetal loss, this rate was 2.9% (2/68). In 18 of the 137 "single needle" TA-CVS performed to 135 patients, the cytogenetic analyses were done by direct method only, in 13 by culture method and in the remaining 104 cases by both methods. The success rate of cytogenetic analysis was 94.5% in the direct method, 92.3% in culture method and 96.2% in the combination of both methods. The overall success rate of cyotogenetic diagnosis in our series was 95.5%. The overall rate of choromosomal abnormalities was determined as 8.15% (11/135). This rate was 7.31% (3/41) in the "advanced maternal age" and 14.7% (5/34) in the "pathological ultrasound" groups. The rate of false positive results due to "confined placental mosicism" was 1.4%. Maternal cell contamination was not observed. Conclusion: Our findings were compared with those in the literature and it was decided that CVS procedures were successfully established in our clinic when both the success rates of the technique and cytogenetic diagnosis are taken into consideration.
