ÖZET
Mikrocerrahide başarılı olmak için iki temel kural vardır. Bunlardan biri hasta seçimi, diğeri de uygulanacak tekniğin seçimidir. Hasta seçimindeki ilkeleri şu şekilde açıklamak mümkündür: Histerosalpingografi ve laparoskopi uygulanarak Tubalar iyi bir şekilde tetkik edilmelidir. Bu tetkiklerdeki görüntü ve bulguların yorumlarının iyi bir şekilde yapılabilmesi için tüberküloz, endometriozis, klamidia, non spesifik enfeksiyonlar ve laparotomilerin tubada yapabileceği patolojiler iyi bilinmelidir. Ayrıca tubadaki patolojinin yerleştiği tüp segmenti ve patolojinin ne zamandan beri mevcut olduğu da önemlidir. Bunlara ilave olarak uygulanan teknik de önemlidir. Tubanın proksimal patolojilerinde implantasyon, Ehrler ya da Trand tekniği uygulanabilir. Medial bölgede klasik teknik, Diamond, Gomel ve Coock tekniklerinden biri seçilebilir. Distal lezyonlarda neostomiler yapılırken Palmer, Polloson veya Cognat teknikleri uygulanabilir. Operatör bu tekniklerden birini, patolojinin tubada yaptığı hasara göre tercih edebilmelidir.
ABSTRACT
There are two basic principles for succes in microsurgery. These are patient selection and the surgicale technique. The work-up in selecting the patient for microsurgery should include the evaluation of the fallopian tubes by hysterosalpingography and laparoscopy. In orden to have accurate comments from the images and the findings of these procedures, one has to know very well about the correlations of tuberculous salpingitis, endometriosis, clamidial and non specific infection and previous laparotomies to tubal pathologies. The anatomic segment of the tubal disease and the time from the occurence of the tubal pathology are also important. Besiders patient selection, the applied surgical technique has also importance. The utero-tubal implantation procedure, Ehrler's and/or Trand's techniques may be performed for proximal tubal lesions. One of the procedures among classical, Diamond's, Gomel's and Cook's techniques can be applied for mid-tubal lesions. For distal tubal lesions salpingoneostomy procedures of Palmer's, Polloson's and Cognat's techniques can be performed. The microsurgeon should have the experience to choose one of these surgical techniques according to the present tubal disease.
