ÖZET
Artrogripozis multipleks konjenita (AMC), doğumsal eklem kontraktürü sendromlarından oluşan heterojen bir gruptur. İki olgunun sunumu ile üçüncü trimesterde polihidramniyos bulunan olgularda AMC tanısına dikkat çekmeyi amaçlıyoruz.  Anneler 26 ve 32. gebelik haftaları arasında kliniğimize yönlendirilmişlerdi. İki boyutlu (2D) ultrasonografide bulgularımız: polihidramniyos, artmış ense kalınlığı, mide cebinin izlenmemesi, hafif ventrikülomegali (bir olguda) ve fetal ekstremitelerin sabit pozisyonu idi. Üç boyutlu (3D) görüntüleme ile fetal ekstremite ve vücudun sabit duruş bozukluğu gösterildi. Dört boyutlu (4D) ultrasonografi ile fetal hareketlerin yokluğu doğrulandı. Gebeliklerin ikisi de 32 ve 41 gebelik haftalarında fetal-neonatal ölüm ile sonuçlandı. Polihidramniyos bulunan bir olguda fetal AMC tanısı konvansiyonel ultrasonografi ile kolaylıkla koyulabilir. Üç boyutlu ultrasonografi ile ebeveynler için daha anlaşılabilir resimler elde edilebilir. Eğer AMC olgusunda polihidramniyos mevcutsa prognoz kötü olabilir.
ABSTRACT
Arthrogryposis multiplex congenita (AMC) is a heterogeneous group of congenital contracture syndromes. We present two cases and attempt to increase the awareness for the diagnosis of AMC in the cases with polyhydramniosis at third trimester.  Two mothers were refereed at 26 and 32 weeks of gestation. The findings by two dimensional (2D)  ultrasonography were: polyhydramniosis, increased nuchal edema, nonvisible stomach, mild ventriculomegaly (in one case) and fixed position of the fetal extremities. Three-dimensional images were used to demonstrate the fixed postural abnormalities of the fetal extremities and body. Four-dimensional ultrasonography was used to confirm the absence of fetal movements. Both of the pregnancies resulted with fetal-neonatal demise at 32 and 41 weeks of gestation.  In a case with polyhydramniosis the prenatal diagnosis of fetal AMC can easily be made by conventional ultrasonography. Three dimensional imaging might provide images which might be more understandable for the parents. In a case of AMC associated with polyhidramniosis the prognosis might be poor.
