Postpartum Dexamethasone Use In Patients With HELLP Syndrome

ÖZET
Amaç: HELLP sendromlu gebelerde postpartum dönemde deksametazon tedavisinin klinik ve laboratuvar parametreleri üzerine tedavi edici etkinliğinin araştırılması.Çalışmanın Yapıldığı Yer: Fırat Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum Anabilim DalıMateryal ve Metod: Antepartum dönemde tipik komplet Class 1-2 HELLP sendromu tanısı konan 27 gebe postpartum dönemde rastgele steroid alan (grup 1=çalışma grubu, n=13) ve almayanlar (grup 2=kontrol grubu, n=13) olarak iki gruba ayrıldı. Bir vaka, doğum anında koryoamnionit olduğu için çalışmadan çıkarıldıÇalışma grubuna postpartum 0. saat ve 12 saat sonra 10 mg deksametazon intravenöz (IV), daha sonra 2x5 mg IV/gün (toplam 30 mg deksametazon) verildi. Kontrol grubuna deksametazon verilmedi. Postpartum ilk 48 saatte klinik ve laboratuar parametreler bakıldı. Hastaların hepsi yoğun bakımda takip edildi. Yoğun bakımdan çıkarılma kriteri olarak 100 ml / saatlik idrar çıkışı, TAin 160/100 mmHg olması kabul edildi.Prospektif, randomize kontrollü, tek kör çalışma uygulandı. Verilerin istatistiksel analizi SPSS 9.0 programı ile nicel ve ordinal verilerde Mann - Whitney U testi (p<0.05 anlamlı) ve grup içi tekrarlı ölçümde Friedman varyans analizinde p<0.05 olması üzerine Wilcoxon Rank testi yapıldı. MAP için p<0.03, idrar itrahı, SGOT, SGPT ve LDH için p<0.025, platelet içinse p<0.02 anlamlı kabul edildi.Bulgular: İki gruptaki hastaların başlangıçtaki klinik ve laboratuvar parametreleri benzer olmasına rağmen (p>0.05, Mann Whitney U Testi), deksametazon tedavisi uygulanan hastalarda hızla düzelme görüldü (MAP p<0.03, idrar itrahı, SGOT, SGPT ve LDH p<0.025, platelet p<0.02, Wilcoxon Rank Testi), eritrosit süspansiyonu, taze donmuş plazma, yoğun bakımda kalma ve taburcu edilme süreleri anlamlı olarak azaldı (p<0.05, Mann Whitney U Testi).Sonuç: Postpartum deksametazon tedavisi alan grupta klinik ve laboratuvar parametrelerinde hızlı iyileşme, daha az morbidite, yoğun bakım tedavisi, yoğun bakımda ve hastanede kalış süresi olduğu saptandı. Postpartum deksametazon tedavisinin HELLP sendromlu vakalarda uygulanması tavsiye edilebilir.
ABSTRACT
Objective: To investigate the therapeutic effects of dexamethasone on clinical and laboratory parameters in the postpartum period of the patients with HELLP Syndrome.Setting: Department of Obstetrics and Gynecology, Firat University, School of MedicineMaterial and Method: Twenty-seven pregnant patients diagnosed as having typical complete Class 1-2 HELLP Syndrome in their antepartum periods were classified as Group 1 (study group) who were randomly chosen to receive steroids in the postpartum period; and Group 2 (control group) who did not. One case was excluded due to chorioamnionitis developed at the time of labor.10 mg of dexamethasone was administered intravenously to the study group immediately postpartum 0 and at the 12th hour, and then 2x5 mg IV/day thereafter ( a total of 30 mg dexamethasone). No dexamethasone was given to the control group. Clinical and laboratory parameters were checked for the first 48 hours in the postpartum period. All of the patients were monitored in intensive care unit (ICU). Discharge criteria from ICU included urine output ³100 ml/hr and TA< 160/100 mmHg.A prospective, randomized controlled and single blinded study was employed. Results were statistically evaluated with SPSS 9.0 program. For quantitative and ordinal data, Mann Whitney U test (p<0.05 is significant) and for intragroup repeated calculations Friedman Variance Analysis (p<0.05) were used, hence we proceded with Wilcoxon Rank Test. For MAP, P<0.03; for urine output, SGOT,SGPT, and LDH, p<0.025; whereas for platelet count, p<0.02 were found to be significant.Results: Despite the similar results obtained in clinical and laboratory parameters for both groups (p>0.05, Mann Whitney U); rapid recovery was observed in dexamethasone-treated patients (MAP p<0.03, urine output, SGOT, SGPT and LDH p<0.025, platelet p<0.02, Wilcoxon Rank Test); the amount of transfused red blood cell, fresh frozen plasma, the length of ICU and/or hospital stay were reduced significantly (p<0.05, Mann Whitney U Test).Conclusion: Dexamethasone-treated group demonstrated rapid recovery in terms of clinical and laboratory parameters and less morbidity in terms of duration of ICU and hospital stay. We concluded that postpartum dexamethasone therapy can decrease morbidity in patient with HELLP syndrome.Key Words: Dexamethasone, HELLP Syndrome
