ÖZET
Kanser tedavilerindeki cerrahi, medikal ve teknolojik gelişmeler artan sağkalım oranları ile birlikte, yaşam kalitesinde iyileşmeye yol açmakta ve fertilitenin korunması ve devamlılığının sağlanması giderek daha büyük önem kazanmaktadır. Fertilite korunmasında seçenekler tanı ve tedavi sırasında hastanın yaşı; kanser tipi ve lokalizasyonu; hastalığın şiddeti ve tedavi tipi ile ilişkilidir. Günümüzde kanser tedavisi konservatif cerrahi, kemoterapi (KT), radyoterapi (RT) ve allojenik kemik iliği transplantasyonu (KİT)'ndan oluşmaktadır ve bu güncel tedavi yöntemleri ile meme kanseri gibi bazı kanserlerde kür oranları %90'ı geçmiştir, ancak agresif KT ve RT tedavi protokolleri sonrası kadın fertilitesini korumak için etkili klinik yöntemler oldukça azdır. Radyasyon veya KT'nin over fonksiyonlarına olumsuz etkileri ilerleyici ve çoğunlukla dönüşümsüzdür, kalıcı amenore ve infertiliteye neden olmaktadır. Özellikle alkilleyici ajanlar ve iyonizan radyasyon prematür over yetmezliğine sıklıkla yol açmaktadır. Onkologların tedaviye başlamadan önce hastalarının over fonksiyonu ve gelecekteki fertilitelerinin korunmasını dikkate almaları büyük önem taşımaktadır. İn vitro fertilazasyon (IVF) uzmanı, jinekolojik radyasyon onkoloğu ve medikal onkoloğun ortak bir konsültasyon ile KT ve/veya RT dozlarını tartışmaları, bu dozların oluşturacağı olası ovaryan hasara ilişkin riskler yönünden hastayı bilgilendirmeleri ve fertilite koruyucu yöntemlerin hastaya anlatılması günümüzde kabul edilen en uygun yaklaşım şeklidir. Kanser saptanan kadınlarda fertiliteyi korumada günümüzde uygulanan ve gelecekte uygulanabilecek farklı yöntemler vardır. Fertilitenin korunması için onaylanan yöntemleri erkekte spermin kriyoprezervasyonu ve kadında yardımcı üreme teknikleri (YÜT) uygulayarak embriyo kriyoprezervasyonu şeklindedir. Oositlerin vitrifikasyon yöntemi ile kriyoprezervasyonu günümüzde büyük önem kazanmıştır ve yakın gelecekte kadın fertilitesini korumada yaygın olarak kullanılması beklenmektedir.
ABSTRACT
Surgical, medical, and technological advances in cancer treatments have enabled to improve the life quality with increased survival rates, making preservation of reproductive function a more important priority than ever before. During diagnosis and treatment, the options of fertility preservation will depend on the age of the patients at the time of diagnosis and treatment, cancer type, severity and location, and type of treatment. With current treatment regimens, including conservative surgery, aggressive chemotherapy (CT), radiotherapy (RT), and bone marrow transplantation (BMT), cure rates from some malignancies like breast cancer can exceed 90%. However, after aggressive chemotherapy and radiotherapy protocols, there are few clinical procedures to protect female fertility. The effect of RT or CT on ovary is progressive and mostly irreversible. Also, these treatment modalities cause permanent amenorrhea and infertility. Especially the alkylating agents and ionizing radiation can often induce premature ovarian failure. Oncologists have to pay great attention protecting the ovarian function and have to consider fertility preservation options in patients undergoing therapy. In vitro fertilization (IVF) specialist, gynaecologic radiation oncologist and medical oncologist should discuss together the dosage of chemotherapy and/or radiotherapy and patients should be acquainted about ovarian damages which will be caused by radio/chemotherapy dosages. Today the most accepted management is to discuss with the patient fertility conservation procedures. At the moment, there are different methods performed to preserve fertility in women diagnosed with cancer and many are promising for the future. The only established methods for fertility preservation are sperm cryopreservation in men and IVF with embryo cryopreservation in women. Oocyte cryopreservation with vitrification technique seems to be the method of female fertility preservation that will be widely used in the near future.
