ÖZET
Amaç: Endometrium kanserlerinde oluşturulan protokole göre erken sonuçlarının değerlendirilmesi. Çalışmanın Yapıldığı Yer: Gülhane Askeri Tıp Fakültesi. Materyel ve Metod: 1990-1994 yılları arasında başvuran 40 endometrium Ca olgusu klinikler arası oluşturulan protokole göre tedavi edilmişlerdir. Tedavi protokolünü; grade, myometrial invazyon, lenf ve vasküler invazyon, pelvik ve paraaortik lenf nod tutulumu belirlemiştir. FIGO'ya göre Evre Ia ve Ib olan 14 olguya cerrahiyi takiben intrakaviter brakiterapi uygulanmıştır (HDR, 500x4= 2000 cGy. TTD). Evre Ic, IIa, IIIa olan 26 olguya eksternal RT ve intrakaviter RT uygulanmıştır (Ex. RT 200x23= 4600 cGy + 750x2= 1500 cGy.; TTD= 6100 cGy). Bulgular: Hastalar ortalama 27.8 ay takip edilmiştir. Bu takiplerde üç olguda akciğer metastazı, bir olguda şüpheli kemik metastazı, bir olguda karaciğer metastazı ve bir olguda da lokal nüks saptanmıştır. Ocak 94'e kadar yapılan takiplerde olguların hepsi hayattaydı. İleri evrede (IIIa) görülen tek lokal nüks dışında lokal kontrol sağlanmıştı. Sonuç: Takip süremizin kısa olmasına karşın nükslerin %80'inin ilk iki yıl içinde ortaya çıkması gözönüne alındığında bu çalışmanın erken sonuçları oldukça yüz güldürücüdür.
ABSTRACT
Objective: Evaluating of early results according to the protocol formed in the endometrium cancers. Institution: Gülhane Military Medical Faculty. Materials and Methods: Fourty cases of endometrium cancers had been treated according to the protocol formed by concerned clinics between 1990-1994. The treatment protocol had been determined in regards to the grade, myometrial invasion, lenf and vascular invasion, pelvic and paraaortic lenf invasion. In 14 cases which were stage Ia and IIb according to FIGO intracaviter brachytherapy had been applied following surgery (HDR, 500x4= 2000 cGy. TTD). In twentysix cases which were Ic, IIa, IIb, IIIa external radiotherapy and intracaviter radiotherapy had been applied (Ex.RT 200x23= 4600 cGy. + HDR 750x2= 1500cGy.; TTD= 6100 cGy). Findings: Patients were followed for 27.8 mounhts average. In this followings in three cases lung metastasis, in one cases suspectious bone metastasis, in one case liver metastasis and in one case regional relapse were found. All cases were alive until January 1994. Regional control had been obtained in all cases but one which were stage IIIa and had regional relapse. Results: Early results of this study were rather good considering the 80% of the relapsing were came in to two years in spite of our following period was short.
