Role Of Interleukin-6 And Renin- Angiotensin Systemin Ovarian Hyperstimulation Syndrome: Rabbit Model

ÖZET
Amaç: Ovarian hiperstimulasyon sendromu etiopatogenezinde interleukin-6 ve renin-angiotensin sisteminin rolünü araştırmak. Çalışmanın Yapıldığı Yer: Dokuz Eylül Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum Anabilim Dalı. Materyel ve Metod: Çalışmaya 1. grupta 10 ve 2. grupta 10 olmak üzere toplam 20 adet dişi Yeni Zelanda tavşanı alındı. Bütün tavşanlara 1-7. gün 75 IU FSH ve 75 IU LH, İM. ve 7. gün 2500 IU HCG İM yapıldı. 1. grup tavşanlara 2mg/kg enalapril 12 saat ara ile oral verildi. 9. gün laparatomi yapıldı ve peritoneal sıvıya, over ağırlığına bakıldı. Her iki grupta 1. ve 9. gün plazma renin, IL-6, estradiol, progesteron, prolaktin ve aldosteron düzeyleri saptandı. Bulgular: Çalışmamızda 9. günde plazma interleukin-6 ve renin düzeylerinin bazal düzeye göre her iki grupta da anlamlı olarak arttığı saptandı. Her iki grupta da 9. gün IL-6 ile renin düzeyleri arasında korelasyon dikkat çekmekteydi. Peritoneal sıvı, 9. gündeki tavşan ağırlık artışı göz önüne alındığında iki grup arasında anlamlı bir fark bulunamamıştır. Enalapril plazma aldosteron düzeylerini düşürmesine rağmen asit oluşumunu engelleyememiştir. Enalapril verilen grupta over ağırlığı anlamlı olarak yüksek bulunmuştur. Sonuç: Ovarian hiperstimulasyon sendromu patogenezinde renin angiotensin sistemi ve interleukin-6 rol oynayabilir. Enalapril kullanımının ovarian hiperstimulasyon sendromu oluşumunda faydalı bir etkisi yok gibi görünmektedir.
ABSTRACT
Objective: To investigate the role of the renin-angiotensin system and Interleukin-6 in the etiopathogenesis of Ovarian Hyperstimulation Syndrome. Institution: Dokuz Eylul University, Faculty of Medicine, Department of Obstetrics&Gynecology. Material and Method: 20 New Zealand female rabbits were included in the study. 75 IU FSH +75 IU LH intramuscular injections were performed daily in all rabbits during the first 7 days. 2500 IU HCG were injected intramuscularly at 7 th day in all rabbits. Oral enalapril tablets were taken additionally with a dose of 2 mg/kg twice daily between the day 0 and 9 in the ten rabbits (Group 1). Other ten rabbits did not receive enalapril (Group 2). Laparotomy was performed at day 9 in all rabbits. The amount of peritoneal fluid and the weights of ovaries were recorded during laparotomy. Plasma renin, interleukin-6, estradiol, progesterone, prolactin and aldosteron levels were assayed at the day 0 and at the day 9 in all rabbits. Findings: Plasma renin and interleukin-6 levels increased significantly at the day 9 compared to basal levels in both groups (p<0,05). Plasma renin levels were correlated with plasma interleukin-6 levels at the day 9 in both groups (p<0,05). The amount of peritoneal fluid and the increase of weight observed at the day 9 were not significantly different between group 1 and 2. Administration of enalapril did not prevent the formation of ascite in group 1 despite the low plasma levels of aldosteron. In group 1 the weight of ovaries was significantly higher compared to group 2 (p<0,05). Conclusion: Renin-angiotensin system and interleukin-6 may play a role in the etiopathogenesis of ovarian hyperstimulation syndrome. Administration of enalapril did not seem to have any beneficial effect on the severity of ovarian hyperstimulation syndrome.
