Seroprevalence Of Hepatitis B, Hepatitis C And Hiv At Whole Obstetric And Gynecologic Patients Who Applied Our Hospital

ÖZET
Amaç: Kesitsel tipteki bu çalışmada, hastanemize başvuran hastalarda hepatit B, hepatit C ve HIV seroprevalansı saptanarak, bu serolojik testlerin gerekliliği araştırıldı. Gereç ve Yöntemler: Hastanemize 2005-2007 yılları arasında başvuran hastalar HBsAg, anti-HCV ve anti-HIV seroprevalansı açısından retrospektif olarak değerlendirildi. HBsAg, anti-HCV ve anti-HIV kemiluminesans immunoassay (CIMA) yöntemi ile incelendi. Şüpheli pozitif bulunan anti-HIV serum örnekleri Western Blot yöntemi ile tekrar çalışılıp doğrulaması yapıldı. Bulgular: HBsAg için incelenen toplam 123 565 kişiden 2 547'sinde (%2.06) HBsAg pozitif olarak bulundu. İncelenenlerin 90 351'i gebe idi. Bu gebelerin 1 910'unda (%2.11) HBsAg pozitif idi. Anti-HCV için incelenen toplam 86 724 kişiden 136'sında (%0.15) anti-HCV pozitif olarak bulundu. İncelenenlerin 60 729'u gebe idi. Bu gebelerin 102'sinde (%0.17) anti-HCV pozitif idi. Anti-HIV için incelenen toplam 86 930 olgunun 4'de (%0.004) anti-HIV pozitif olarak bulundu. İncelenenlerin 60 562'i gebe idi. Bu gebelerinde 3'de anti-HIV pozitif bulunmuş olup, gebelerin tümünde test pozitifliği tarama sırasında saptanmıştır ve hastaların hiçbirinin serolojileri hakkında bilgileri bulunmamakta idi. Bu hastalar asemptomatik olup gebelikleri ekstrauterin gebelik ve abortus ile sonuçlanmıştır. Sonuç: Araştırmada bulduğumuz HBsAg, anti-HCV, anti-HIV seropozitif hasta sıklıkları, ülkemizdeki genel popülasyon için tespit edilen pozitiflik sıklıkları içerisindedir. Bu oranlar göz önüne alındığında herhangi bir nedenle hastaneye başvuran hastalarda bu testler tarama amaçlı olarak en azından insanların hayatlarında bir kez bakılacak şekilde kullanılabilir. Gebeler ayrı bir kategoride değerlendirilip bu testler rutin gebelik takibinde yerini almalıdır.
ABSTRACT
Objective: In this cross-sectional study, we aimed to determine the seroprevalence of hepatitis B, hepatitis C and HIV at patients who applied to our hospital and to investigate the necessity of these serologic tests. Material and Methods: Patients who applied to our hospital between 2005-2007 were evaluated retrospectively in order to find out the seroprevalence of hepatitis B, hepatitis C and HIV. HBsAg, anti-HCV and anti-HIV were analyzed in the sera of patients via chemiluminescent immunoassay (CIMA). Results: For HBsAg, 123 565 samples were analyzed and 25 47 of these samples were positive (%2.06). Of the patients 90 351 were pregnant, of whom 1 910 (%2,11) were found to be HBsAg positive. For anti-HCV, 86 724 samples were analyzed and the positive rate was %0.15(136/86724). Of the patients 60 729 were pregnant, of whom 102 (%0.17) were found to be anti-HCV positive. For anti-HIV, 86 930 samples were analyzed and found 4 reactive sera which were positive by the confirmation test, too. Of the patients 60 562 were pregnant, of whom 3 were found to be anti-HIV positive. The seropositivity of the samples was determined in the course of screening for all pregnant women and none of them had any opinion about their serology. All the pregnants who were seropositive were asymptomatic and their pregnancies turned out to be ectopic pregnancy and abortion. Conclusion: The seropositivite ratios of HBsAg, anti-HCV and anti-HIV in these patients were not different than those of the nationwide statistics. If these ratios are considered, it is necessary to use these tests for screening HBV, HCV and HIV in patients, who apply to the hospital for any reason, at least once in their lives. Pregnants should be evaluated in different category and routine antenatal screening should be included these tests.
