Thyroid Function Disturbances In Third Trimester Pregnancy

ÖZET
Amaç: 3. trimesterdeki gebelerde tiroid fonksiyon bozuklukluğu insidansını saptamak ve tiroid fonksiyon bozuklukları ile gebelik komplikasyonları arasındaki ilişkiyi araştırmak. Gereç ve Yöntemler: Hastanemize son bir yılda travayda başvuran 3. trimesterde 250 gebe ile gebe olmayan 150 sağlıklı hastada, tiroid fonksiyon testleri yapıldı. Gebelerin ve gebe olmayanların test sonuçları karşılaştırıldı. Tiroid fonksiyon bozukluğu (hipotiroidizm-hipertiroidizm) saptanan gebeler ile sağlıklı gebelerde; preeklampsi, intrauterin gelişme geriliği (IUGR), anemi, erken doğum gibi komplikasyonların görülme sıklığı karşılaştırıldı. Ayrıca hipotiroidili ve hipertiroidili gebeler, kendi aralarında bu komplikasyonların görulme sıklığı açısından tekrar karşılaştırıldı. Bulgular: Son trimester gebelerin %5.6'sında tiroid fonksiyonlarında bozukluk saptandı. %1.6 oranında hipotiroidizm ve %4 oranında hipertiroidizm saptandı. Bu oranlar literatürden yüksekti. Tiroid fonksiyonları bozuk olan gebelerde en sık preeklampsi (%57) ve IUGR (%36) saptandı. Prematür doğum oranı %7 olup literatürden oldukça düşüktü. Bu komplikasyonların görülme sıklığı açısından hipotiroidizm ve hipertiroidizm arasında fark yoktu. Hipotiroidili gebelerde ek olarak, %25 oranında anemi tespit edildi. Sonuçlar: Gebelikte görülen hipotiroidizm-hipertiroidizm prevalansı ülkemizde, diğer ülkelerden yüksek olabilir. Sonuç olarak; tedavi edilmeyen tiroid fonksiyon bozuklukları; maternal ve fetal mortalitenin yüksek olduğu preeklampsi, IUGR ve erken doğum gibi komplikasyonlara yol açar.
ABSTRACT
Objective: To investigate the incidence of thyroid function disturbances in third trimester pregnancies and also to investigate relation of thyroid function disturbances with complications seen in third trimester. Material and Methods: Thyroid function tests were performed in 250 pregnant women attended for labor to our hospital during last year (in third trimester) and 150 healthy non pregnant women. Pregnant women with thyroid function disturbances (hypo-hyperthyroidism) were compared with healthy pregnant women for the complication frequencies such as preeclampsia, IUGR, premature labor. Pregnant women with hypothyroidism and hiperthyroidism were compared also for the frequencies of the complications mentioned above. Results: 5.6% of last trimester pregnancies had thyroid function disturbances. Hypothyroidism were diagnosed in 1.6% of these pregnants and hyperthyroidism in 4% of them.These incidences were higher than literature. The most frequent complications were preeclampsia (57%) and IUGR (36%). These rates were higher than other studies in literature. Premature labor was seen in 7% of patients which was lower than other studies. There was no difference between hyperthyrodic pregnants and hypothyroidic pregnants considering frequencies of complications. In 25% of hypothyroidic pregnants, anemia was also found. Conclusions: The prevalance of hypothyroidism and hyperthyroidism seen in pregnancy may be higher in our country comparing to other countries. In conclusion; these pregnancies if no treatment has been given are prone to complications in high rates such as preeclampsia, IUGR and early labor which in turn leads to high maternal and fetal mortality.
