Factor V Leiden Mutation In A Patient Recurrent Pregnancy Loss and Deep Vein Thrombosis

ÖZET
Amaç: Bu makalede tekrarlayan gebelik kayıpları ve geçirilmiş derin ven trombozu nedeniyle 7. gebeliğinde kliniğimizde takibe alınan bir hastada, faktör V Leiden mutasyonunun saptanması ve bu durumun etiyolojideki rolü sunulmaktadır.Olgu Sunumu: Tekrarlayan gebelik kayıpları ve geçirilmiş derin ven trombozu öyküsü nedeni ile 7. gebeliğinin 16. haftasında kliniğimizde takibe alınan bir hasta sunulmuştur. Ultrasonografide gebelik haftası ile uyumlu bir adet canlı fetus izlenmiştir. Koagülasyon taramasında protein S aktivitesi düşük, antifosfolipid antikor taraması negatif, DNA polimeraz zincir reaksiyonu ile çalışılan faktör V Leiden mutasyonu heterozigot pozitif olarak saptanmıştır. Doğum sezaryen ile termde gerçekleştirilmiş ve bir adet sağlıklı bebek doğurtulmuştur. Erken puerperal dönemde sol bacakta tromboz gelişen hastaya düşük moleküler ağırlıklı heparin ile antikoagülasyon başlanmıştır.Sonuç: Faktör V Leiden mutasyonu gebelik ve puerperium döneminde gelişen derin ven trombozu ve tekrarlayan gebelik kayıplarda etiyolojik faktör olarak yer alabilir.
ABSTRACT
Objective: In this paper a pregnant patient, gravida 7, para 0, with a history of recurrent pregnancy loss and deep vein thrombosis was presented.Case Report: The patient presented on her 16th gestational week and ultrasonography revealed a viable fetus compatible with the gestational week age. In coagulation screening the activity for protein C was low, and antiphospholipid antibody screening was negative. Factor V Leiden mutation assessed by DNA polymerase chain reaction was found to be heterozygote positive. A healthy term infant was delivered by cesarean section. In the early puerperium deep venous thrombosis occurred in the left leg and treated with low molecular weight heparin.Conclusion: Factor V Leiden mutation may be a responsible factor in the etiology of recurrent pregnancy loss and deep vein thrombosis that occurs during the pregnancy and puerperium.
