Tension-free Vaginal Tape Procedure In Surgical Treatment Of Stress Incontinence: Short Term Results

ÖZET
Amaç: Bu çalışmanın amacı TVT yönteminin teknik yönlerinin tartışılması ve TVT uygulanan olguların kısa dönem sonuçlarını sunmaktır.Metod: 1999-2001 yılları arasında stres üriner inkontinansı olan 17 olguya TVT operasyonu uygulandı. Bu olgulara ait, operasyon süresi, kateterizasyon süresi, ortalama hastanede kalış zamanı, intraoperatif ve postoperatif komplikasyonlar ile operasyonun başarısı değerlendirildi.Bulgular: 17 olgunun 12sinde anatomik tipde stres inkontinans, 3 olguda intrensek sfinkter yetmezliği, 2 olguda ise nüks inkontinans mevcuttu. Uygulanan toplam 17 TVT operasyonunda 13 olguya yalnızca TVT uygulaması yapılırken, 3 olguda TVT operasyonu vajinal histerektomi ve kolporafi anterior-posterior ile 1 olguda ise ve vajinal histerektomi ve kolporafi anterior-posterior, sakrospinöz fiksasyon operasyonu ile kombine edildi. Olguların ortalama yaşı 51.6 (38-75 yaş) idi. Ortalama operasyon süresi 34 dk, ortalama üriner kateterizasyon süresi 3.04 gün, ortalama hastanede kalış süresi 3 gün idi. Operasyon sırasında 2 olguda mesane yaralanması, 1 olguda ise üretra yaralanması meydana geldi. Postoperatif dönemde ise 2 olguda idrar yolu enfeksiyonu ve idrar retansiyonu ile 1 olguda flegmon-abse formasyonu gelişti. Bir olguda de novo urge inkontinans ortaya çıktı. Bu olgu dışında postoperatif ortalama 15 aylık izlem boyunca olguların hiçbirinde postoperatif izlemlerde rekürrens saptanmadı.Tartışma: TVT operasyonu anatomik stres inkontinans olguları kadar nüks ve intrensek sfinkter yetmezliği olan olgularda da etkin bir şekilde uygulanabilecek bir yöntemdir.
ABSTRACT
Objectives: The purpose of this study is to discuss the technical aspects of TVT procedure and to present our short term results.Methods: TVT procedure was performed on 17 patients with stress urinary incontinence between 1999 and 2001. Operation time, duration of urinary catheterisation, mean hospital stay, intraoperative and postoperative complications were evaluated.Results: 12 of 17 cases had anatomic type stress incontinence, 3 had intrinsic sphincter deficiency and 2 have recurrent incontinence. We performed 17 TVT operations. In 13 patients TVT procedure was performed alone but in 3 patients TVT was combined with vaginal hysterectomy and colporaphy anterior and posterior and in one patient TVT was combined with vaginal hysterectomy and colporaphy anterior and posterior and sacrospinous fixation operation. The mean age was 51.6 (38-75 range). Mean operation time was 34 minutes, mean duration of urinary cathaterisation was 3.04 days and mean hospital stay was 3 days. Two bladder injury and one uretheral injury occured during the TVT procedure and two urinary tract infection and urinary retantion developed in postoperative period. Mean follow up time was 15 months. In another patient abcess formation occured in postoperative period. De novo urge incontinence developed in one patient. We detected no stress urinary incontinence during postoperative period except one who developed de novo urge incontinence.Conclusion: These results show TVT is not only an effective procedure for surgical treatment of anatomical stress urinary incontinence but effective in intrinsic sphincter deficiency and recurrent stres incontinence.
