ÖZET
Amaç: Bu çalışma; uzun süreli kist hikayesi, normal tümör marker seviyeleri, oral kontraseptif (OK) tedaviye rezistans ve doppler ultrasonografide artmış pulsatil indeksten (PI) oluşan dört parametrenin paraovaryan kistik lezyonların preoperatif tanısında ki yerini saptamak için planlandıMateryel ve Metod: Jinekolojik bir nedenle laparatomi/laparaskopi yapılan 145 hasta retrospektif olarak incelendi. Hastalara preoperatif konulan tanılar ile intraoperatif ve postoperatif tanılar karşılaştırıldı. Postoperatif paraovaryan kistik kitle tanısı alan hastaların, şikayet süreleri, tümör markerları, kitlenin doppler bulguları ve OK tedaviye cevap verip vermemesi birlikte değerlendirilerek yeniden tanıya gidildi ve ilk tanılar ile son tanıların doğruluk oranları karşılaştırıldı.Sonuçlar: Hastaların postoperatif tanıları ile preoperatif tanıları karşılaştırıldığında, 145 hastanın 122sinin over kökenli, 6sının subseröz myom, 17 olgunun ise paraovaryan orijinli olduğu saptanmıştır. On yedi hastadan sadece 2 hastaya preoperatif paraovaryan kitle ön tanısı konmuş ve postoperatif olarakta bu doğrulanmış, 15 olguda (%85.8) ise tanı atlanmıştır. Nonovaryan adneksial kitleli 17 hasta retrospektif olarak kendi arasında OK tedavisine cevabına, semptom süresine, tümör markerlarına ve doppler sonografi bulgularına göre retrospektif olarak değerlendirildiğinde 13 olguya (%71.4) preoperatif olarak doğru tanı konmuştur.Tartışma: Uzun süredir kist hikayesi olan, doppler sonografide artmış PI gösteren, OK tedavisine rağmen boyutlarında küçülme olmayan ve tümör markerları normal bulunan adneksial kitlelerde ön tanıda mutlaka paraovaryan kistik kitle de düşünülmelidir.
ABSTRACT
Objective: Preoperative diagnosis of paraovarian cystic lesions may be achieved by a history of long duration, normal levels of tumor markers, unresponsiveness to the oral contraceptive (OCS) treatment and presence of increased pulsatile index (PI) in doppler ultrasonography. In this study we aimed to evaluate the role of above mentioned four parameters to diagnose correctly paraovarian adnexial masses.Material and Method: One-hundred fourty five patients that underwent laparotomy/laparoscopy, for a gynecologic condition have been reviewed retrospectively. Preoperative versus intra and postoperative diagnosis of these 145 patients were compared. The previous history duration of cyst duration, levels of tumor markers, doppler sonographic properties of the lesions and the size of the cyst before and after OCS treatment were evaluated together and final diagnosis compared with postoperative diagnosis.Results: Histopathological findings of the 145 patients revealed that 122 were ovarian, 17 paraovarian, 6 fibroids. It has been found that only two of 17 patients could have been correctly diagnosed preoperatively, 15 cases (85.8%) have been misdiagnosed. Interestingly, when these 17 patients with paraovarian masses were evaluated retrospectively according to their responsiveness to OCS treatment, duration of symptoms, levels of tumor markers and doppler sonographic findings, it has been found that 13 of 17 patients (71.4%) would have been correctly diagnosed preoperatively.Conclusion: We speculate that when there is peripheral angiogenesis and high pulsatıle index at the doppler sonography, long duration of cyst history, resistance to the OCS treatment and normal levels of tumor markers of adnexial cystic masses a paraovarian cyst should be considered.
