ÖZET
Amaç: Abdominal myomektomi vakalarında intraoperatif bilateral a uterina ascendens ligasyonu ile turnike yöntemi kullanımının, intraoperatif ve postoperatif hemostaz üzerine etkilerinin karşılaştırılması.Çalışmanın Yapıldığı Yer: Fırat Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum ABD. Elazığ.Materyal ve Metod: Yirmibeş semptomatik myoma uterili vaka prospektif, rastgele, tek kör iki gruba ayrıldı. Operasyon hazırlığını takiben çalışma grubuna (n=13) intraoperatif bilateral A. Uterina ascendens ligasyonu ve myomektomi, kontrol grubuna ise (n=12) intraoperatif turnike yöntemi uygulanarak myomektomi yapıldı. Nicel ve ordinal verilerin karşılaştırılmasında Mann Whitney U testi, nominal verilerde X2 ve tekrarlı ölçüm yapılan verilerde Wilcoxon Rank testleri kullanıldı, p<0.05 anlamlı kabul edildi.Bulgular: Her iki grupta operasyon süresi, çıkarılan myom nüve sayısı, kan kaybı (p>0.05, Mann Whitney U testi) ve postoperatif myom rekürrensi, febril morbidite benzer bulundu (p>0.05, Fisher exact test).Sonuç: Abdominal myomektomi vakalarında bilateral a uterina ascendens ligasyonu ile turnike yönteminin hemostaz üzerine benzer etkinliği olmasına rağmen, ligasyon kalıcı olduğu için postoperatif dönemde hemostaz üzerine etkinliği devam eder. Operasyon bitiminde turnike çekildiği için postoperatif hemostaz üzerine turnike yönteminin etkinliği yoktur. Ayrıca myomektomileri takiben ortaya çıkan postoperatif hemoraji vakalarında acil laparatomiden başka seçenek bırakmadığı için bilateral A. Uterina ascendens yöntemi tercih edilebilir.
ABSTRACT
Object: To compare the effects of bilateral ligation technique of ascending uterine arteries with tourniquet method on hemosthasis intraoperatively and postoperatively in abdominal myomectomy cases.Setting: Department of Obstetrics and Gynecology, School of Medicine, Firat University,Elazig.Material and Method: Twenty-five symptomatic patients with myoma uteri were randomly divided into two groups. All of the patients were prepared for operation and myomectomies were performed after intraoperative bilateral ligation of ascending uterine arteries in the study group (n=13), whereas in the control group (n=12) after intraoperative tourniquet method. In the assessment of quantitative and ordinal data, Mann Whitney U test was employed in the study group (n=13), whereas in nominal data X2 test, and in repeated calculations, Wilcoxon Rank test were used in which p<0.05 was accepted to be significant.Result: All results were found to be similar in terms of length of operation, number of myoma nuvea., blood loss (p>0.05, Mann Whitney U test) and postoperative myoma recurrence, febrile morbidity in both groups (p>0.05, Fischer exact test).Conclusion: In abdominal myomectomy cases, despite the similar effects of bilateral ligation technique of ascending uterine arteries and tourniquet method on hemosthasis, effect of the former is longlasting as it is a permanent procedure. Since tourniquet was removed at the end of operation, no longer hemosthatic effects could be observed. Besides, in case of postoperative bleeding after myomectomy procedure, since emergency laparotomy is the only procedure to be done; ligation method can be preferred.
