THE CONCENTRATION OF PLASMA ? -ATRIAL NATRIURETIC PEPTIDE IN PREGNANCY

ÖZET
Amaç: Normal ve volüm yüklenmiş gebeliklerde plazma ?-Atrial Natriüretik Peptid (?-ANP) düzeylerinin araştırılması. Çalışmanın Yapıldığı Yer: Gazi Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum AD. Materyel ve Metod: Toplam 58 kadın çalışmaya alındı, 10 tanesi volüm yüklenen, 24 tanesi normal gebe ve 24 tanesi de kontrol grubunu oluşturdu. Gruplar arasında yaş, gebelik haftası ve parite açısından fark yoktu (p> 0.05). 10 tane gebeye 1 saat süreyle 1000 cc %0.9 NaCl infüzyonu yapıldı. Her üç gruptan supin pozisyonda ön koldan alınan venöz kanlar, 1 mg/ml EDTA (Etilen Diamin Tetra Asetilkasit) ve 500 KIU/ml Aprotinin (Trasylol) içeren plastik tüplerde toplandı. 2000 g 30 dk soğuk santrifüj ile ayrılan plazmalar -30oC'de saklandı. ? -ANP düzeyleri ekstraksiyon işlemi ilavesi ile Radio İmmüno Assay (RIA) ile tayin edildi. Bulgular: Plazma ? -ANP düzeyleri, gebe grubunda 15.1 ± 8.0 (A.Ort ± S.Sapma) pg/ml, kontrol grubunda 14.9 ± 8.1 pg/ml ve volüm yüklenen grupta 48.1 ± 9.1 pg/ml olarak tespit edildi. Kontrol ve gebe grubu arasında fark yoktu (p> 0.05). Buna rağmen, volüm yüklenen gebe grubunda yüksek ? -ANP düzeyleri tespit edildi (p< 0.001). Normal gebeliklerde, farklı gebelik haftalarında plazma ? -ANP düzeylerinin değişmediği tespit edildi (p>0.05). Sonuç: Gebelik esnasında, kontrol grubuna göre plazma ? -ANP düzeylerinde artış gözlenmemiştir. İlave olarak, gebelik ilerlerken plazma ? -ANP düzeyleri artmamıştır. Bununla birlikte, volüm yüklenen gebeliklerde yüksek ? -ANP düzeyleri tespit edilmiştir.
ABSTRACT
Objective: Investigation of plasma ? -ANP levels in normal and volume loaded pregnancies. Institution: Department of Gynecology and Obstetrics, Gazi University Medical Faculty. Material and Methods: Fifty-eight women were included in our study, 10 being volume loaded, 24 normal pregnancies and 24 controls. There was no difference between the two groups with regard to age, parity ad duration of pregnancy (p> 0.05). Ten patients were given 1000 ml, %0.9 NaCl for an hour and blood was taken from the forearm of all patients in the supine position. Then, all samples were collected in the special tubes containing 1 mg/ml EDTA and 500 KIU/ml Aprotinin (Trasylol), and samples were cold-centrifuged at 2000 g for 30 min and plasma were preserved at -30oC. ? -ANP levels in the samples were determined by extraction and RIA method. Findings: Plasma ? -ANP values for different groups were as follows; 15.1 ± 8.0 (Mean ± SD) pg/ml in the pregnancies group, 14.9 ± 8.1 pg/ml in the control group and 48.1 ± 9.1 pg/ml in the volume loaded pregnant group. There was no statistical difference between control group and pregnant group (p< 0.05). However, high ? -ANP values were observed in the volume loaded pregnant group (p< 0.001). It was seen that there was no difference among the ? -ANP levels in the normal pregnancies with regard to gestation weeks (p> 0.05). Results: Plasma ? -ANP levels were found not to be high during pregnancy in comparison with the control group. In addition, the values did not increase as the pregnancy proceeded. Nevertheless, high ? -ANP levels were identified in the volume loaded pregnancies.
