ÖZET
Amaç: Gestasyonel proteinürik hipertansiyonlu hastalardaki idrar protein ve kreatinin ekskresyonunun değişkenliğini ve bunların idrar parametreleriyle aralarındaki ilişkiyi saptamak. Materyal ve Metod: Gestasyonel proteinürik hipertansiyonlu 20 hastadan spot ölçüm için idrar alındıktan sonra tüm hastalardan sekizer saatlik idrar örnekleri toplandı. İdrar parametreleri olarak idrar hacmi, protein ve kreatinin ekskresyonu analiz edildi. İstatistiksel analiz yöntemi olarak tek yönlü varyans analizi ve varyasyon katsayısı kullanıldı. Bulgular: Sekizer saatlik idrar örneklerinde varyasyon katsayısı idrar hacmi için %22, ekskrete edilen protein miktarı için %15 ve ekskrete edilen kreatinin miktarı için %11 olarak bulundu. Sekiz saatlik idrar parametrelerinin 24 saatlik idrar parametreleriyle korele olduğu ve spot ölçümdeki protein ekskresyonunun sekiz ve 24 saatlik protein ekskresyonunu anlamlı derecede gösterebildiği bulundu. Sonuç: Gestasyonel proteinürik hipertansiyonlu hastaların takibi sırasında 24 saatlik idrar parametrelerinin araştırılması halen en güvenilir değerlendirme şekli olmakla beraber bu çalışmada öncelikli ve acil durumlar için 8 saatlik idrar parametrelerinin ve spot ölçümdeki protein ekskresyonunun 24 saatlik idrar parametreleriyle uyumlu olduğu ve spot idrar protein tayininin hasta takibinde güvenle kullanılabileceği sonucuna varıldı.
ABSTRACT
Objective: To determine the variability of protein and creatinine excretion and the correlation between spot and eight hour urinary parameters with 24 hour urinary parameters in gestational proteinuric hypertensive patients. Materials and methods: Spot and three specimens of urine, with eight hour periods were collected from 20 gestational proteinuric hypertensive patients. The urinary volume, the amount of protein and creatinine excretred were measured and analyzed. Results: The coefficient of variation for the eight hourly specimens were for the urinary volume %22, amount of protein excreted %15, amount of creatinine excreted %11. The eight hourly urinary parameters predicted the 24 hourly; the spot urinary protein amount predicted the eight and 24 hourly protein excretion. Conclusion: The analysis of a 24 hour urinary specimen remains as the standard method for determining the degree of proteinuria in pregnancy. The spot urinary protein amount and eight hourly urinary parameters could be used in special conditions.
