The Cytomegalovirus Infection, Which Caused Fetal Death at Second Trimester: Case Report

ÖZET
Sitomegalovirüs (CMV) enfeksiyonunun insidansı gelişmekte olan ülkelerde %0,15-2'dir ve konjenital enfeksiyonun en yaygın nedenidir. Gebelikteki CMV enfeksiyonlarının çoğu asemptomatiktir, ancak fetüs ve yenidoğanda ciddi hastalığa yol açabilir. CMV enfeksiyonlarının tanısı esas olarak virüsün veya virüs komponentlerinin patolojik materyalde direkt gösterilmesi veya indirekt olarak seroloji ile mümkündür. İntrauterin fetal ölümle maternal CMV enfeksiyonu ilişkisi tartışmalıdır. Bu yazıda, fetal histolojik CMV enfeksiyonu nedeniyle, 17. gebelik haftasında fetal ölüm ile sonuçlanan bir olguyu sunduk. Erken ikinci trimesterde fetal ölüm nedeni, primer CMV enfeksiyonu olabilir. CMV'ye karşı bir aşı yoktur. Virüs tükürük veya idrarla kirlenmiş nesneler üzerinde 48 saate kadar bulaşıcıdır, ancak sabunla etkisizleştirilebilir. Bu nedenle gebeler basit hijyen önlemleri ile enfeksiyon riskini azaltabilirler.
ABSTRACT
The human cytomegalovirus (CMV) is universally distributed among human populations as one of the most common cause of congenital infection with an incidence of about 0.15'2% in developed countries. Although, most CMV infections in pregnancy are asymptomatic, they may lead to significant problems in fetus or newborn. Diagnosis of CMV infections can be made through directly observing or indirectly by serology of virus or virus components in the pathologic sample. The relationship between fetal death and maternal CMV infection is controversial. In this case report we present a case of stillbirth at 17th week of gestation, caused by a fetal histological CMV infection. Primary CMV infection might be the reason for fetal death at early second trimester. There is no vaccine available against CMV. The virus is transmitted through saliva or dirty objects and remains infectious for 48 hours. However, the virus can be deactivated by soap and therefore pregnant women can minimize the risk of infection by following basic hygene practices.
