The Effect Of Intrapartum Infusions Of Different Oxytocin Dilution On Newborn Serum Bilirubin Levels
İntrapartum Farklı Dilüsyonlarda Oksitosin Kullanımının Yenidoğan Bilirubin Değerleri Üzerine Etkisi

ÖZET
Amaç: Eylem indüksiyonu ile intrapartum oksitosin kullanımının yenidoğan bilirubin düzeyleri üzerine olan etkisinin incelenmesi amaçlandı. Gereç ve Yöntemler: Prospektif olarak planlanan çalışmada Kasım 2000-Aralık 2001 tarihleri arasında toplam 54 gebeden, spontan normal vajinal doğum yapan 23 olgu grup I (kontrol), eylem amacıyla indüksiyon verilen ve toplam <1500 mIU oksitosin kullanılılıp normal vajinal yolla doğum yapan 15 olgu grup II, toplam infuzyonda >1500 mIU oksitosin kullanılan ve normal vajinal yolla doğum yapan 16 olgu ise grup III olarak değerlendirildi. Yenidoğan bebeklerden doğum sonrası ikinci ve dördüncü günlerde topuk kapiller kan örneği alınarak spektrofotometrik metodla serum total bilirubin değerleri mg/dl olarak ölçüldü. Bulgular: İkinci gün yenidoğan serum total bilirubin değerleri grup I, II ve III'de sırasıyla 5.6 ± 2.3 mg/dl, 6.0 ± 1.3 mg/dl ve 6.5 ± 2.8 mg/dl olup aralarında istatistiksel olarak anlamlı fark saptanmadı. Dördüncü gün yenidoğan serum total bilirubin değerleri ise grup I, II ve III'te sırasıyla 7.5 ± 4.0 mg/dl, 10.5 ± 4.8 mg/dl ve 12.6 ± 4.5 mg/dl olarak saptandı (p< 0.0001). Sonuç: Doğum eylemi indüksiyonu amacıyla toplam oksitosin infüzyonu >1500 mIU kullanılan olguların bebeklerinde postpartum 4. gün bilirubin değerleri daha yüksek bulunmuştur.
ABSTRACT
Objective: Aim of the study was to investigate the effect of different dilution of intrapartum oxytocin application for labor induction to the levels of newborn total serum bilirubin. Material and Methods: This study was planned prospectively about total 54 newborns born between November 2000 and December 2001. Fifty-four newborns were investigated in three groups. The first group was the control group (n= 23) and oxytocin was not used for labor induction. In the second group (n= 15) and in the third group (n= 16), a total dose of <1500 mIU and >1500 mIU oxytocin was used for labor induction respectively. Serum total bilirubin levels were obtained by taking capillary blood from the newborns' heel in the second and fourth postpartum days and studied by spectrometric method as mg/dl. Results: Means of total serum bilirubin levels of each group belonging to second day were found to be as 5.6 ± 2.3 mg/dl, 6.0 ± 1.3 mg/dl and 6.5 ± 2.8 mg/dl respectively and there was no statistically significant difference between groups. Means of total serum bilirubin levels of each group belonging to fourth day were determined as 7.5 ± 4.0 mg/dl, 10.5 ± 4.8 mg/dl and 12.6 ± 4.5 mg/dl respectively (p< 0.0001). Conclusion: Newborns delivered by mothers inducted with total dose of >1500 mIU had significantly higher levels of bilirubin on post partum fourth day.
