The Effects of Maternal and Fetal Thrombophilia in Pregnant Women with Preeclampsia and Intrauterine Growth Retardation

ÖZET
Amaç: Preeklampsi ve intrauterin büyüme geriliği olgularında maternal ve fetal trombofili aranarak bu hastalıkların oluşumundaki etkilerini araştırmaktır. Gereç ve Yöntemler: Çalışmaya her bir yarısı çalışma ve kontrol grubunda olacak şekilde 100 hasta dahil edildi. Çalışma grubu  preeklamptik gebelerden oluşurken, kontrol grubunda  sağlıklı gebeler bulunmaktaydı. Çalışma grubu da gelişme geriliğinin olup olmamasına göre iki alt gruba ayrılmaktaydı. Gebelerde serum vitamin B12, folat ve homosistein düzeyleri incelendi. Ayrıca gebelerden ve göbek kordonundan alınan kan örneğinden faktör V Leiden, MTHFR 677, MTHFR 1298 ve protrombin 20210 mutasyonları bakılarak trombofili araştırması yapıldı. Mutasyon analizleri için alınan kanlar örneklerinde DNA izolasyonu yapıldı. Light cycler aleti ile gerçek zamanlı polimeraz zincir reaksiyonu uygulandı ve izole edilen DNAlarda polimorfizm analizi yapıldı. Serum vitamin B12 ve folat, elektrokemilunesans yöntemiyle modüler sistemde incelendi. Homosistein düzeyleri ise yüksek basınçlı sıvı kromotografi (HPLC) yöntemiyle ölçüldü. Bulgular: Gruplar arasında anne yaşı (p= 0.329), gebelik sayıları (p= 0.239) ve doğum sayıları (p= 0.679) bakımından istatistiksel olarak anlamlı farklılık bulunmamaktaydı. Doğumda gebelik haftası (p< 0.001) ve doğum ağırlığı (p< 0.001) açısından gruplar arasında istatistiksel olarak anlamlı derecede fark vardı. Çalışma ve kontrol grupları arasında vitamin B12 (p= 0.124) ve folat (p= 0.609) düzeyleri açısından fark bulunmazken, serum homosistein (p< 0.001) düzeyleri arasında istatistiksel fark bulunmaktaydı. Her iki grup arasında hem fetal, hem de maternal faktör V Leiden, MTHFR 677, MTHFR 1298 ve protrombin 20210 mutasyonları açısından istatistiksel olarak anlamlı fark bulunmamıştır. Sonuç: Maternal ve fetal trombofili varlığı preeklampsi ve intrauterin büyüme geriliği oluşumu ile ilişkili görünmemektedir. Ancak annede hiperhomosistenemi varlığı preeklampsi gelişme riskinde artışla beraber bulunmaktadır.
ABSTRACT
Objective: Our aim was to investigate the effects of the maternal and fetal thrombophilia in the cases of the preeclampsia and intrauterine growth retardation. Material and Methods: "A total of 100 women were included in the study each half was in the study and control groups. While the study group was formed by the women with preeclampsia, healthy pregnants were in the control group. Study group was divided in two subgroups whether the presence of growth retardation. Serum vitamin B12, folate and homocysteine levels were evaluated in pregnants. Additionally, thrombophilia status was examined from the blood samples obtained by the pregnant women or umblical cord by measuring factor V Leiden, MTHFR 677, MTHFR 1298 and protrombin 20210 mutations. DNA isolation was made from the blood samples taken for mutation analysis. Real time polymerase chain reaction was applied using the Light cycler tool and polymorphism analysis was made from the isolated DNAs. Serum vitamin B12 and folat were analyzed using the electrochemiluminescence technique with a modular system. Homocysteine levels were measured using the (high pressure liquid chromatography (HPLC) technique. Results: There was not a statistically significant difference between the maternal age (p= 0.329), number of pregnancies (p= 0.239) and number of deliveries (p= 0.679). On the other hand gestational age at delivery (p< 0.001) and birth weight (p< 0.001) were statistically significant between the groups. While there were not a statistically significant difference between the groups regarding the vitamin B12 (p= 0.124) and folat (p= 0.609) levels, serum homocysteine (p< 0.001) levels were significantly different. When taken into consideration the factor V Leiden, MTHFR 677, MTHFR 1298 and protrombin 20210 mutations there were not any difference in fetal and maternal samples between the groups. Conclusion: The presence of fetal and maternal thrombophilia seems to be unrelated from the preeclampsia and intrauterin growth retardation. However, maternal hyperhomocysteinemia is in relation with the risk of preeclampsia development.
