The Effects Of Tibolone Therapy On Cognitive Functions In Postmenopausal Women

ÖZET
Amaç: Post menopazal dönemde tibolonun bilişsel işlevler ve demans üzerindeki olumlu etkilerini açıklayan bir kaç olası nörobiyolojik mekanizma bulunmaktadır. Bu mekanizmalar östrojen alfa ve/veya beta reseptörlerinin uyarımı, nörotransmitterlerin düzenlenmesi, nöronlar üzerine doğrudan etkiler, lipoproteinlerin düzenlenmesi ve serebral iskeminin önlenmesi olarak sıralanmaktadır. Ancak tibolonun post-menopozal dönemdeki kadınlarda kullanımının bilişsel işlevler üzerine etkilerini araştıran gözlemsel çalışmalardan elde edilen sonuçlar tartışmalıdır. Bu çalışmada, sağlıklı post-menopozal dönemdeki kadınlarda tibolonun bilişsel işlevler üzerindeki etkisini değerlendirmek amaçlanmıştır. Gereç ve Yöntemler: Kadın Hastalıkları ve Doğum Anabilim Dalı menopoz polikliniğinde izlemde olan, sağlıklı 40 olgu (tibolon alan 20 olgu ile, tibolon almayan 20 olgu) çalışmaya alındı. Tibolonun bilişsel işlevler üzerindeki etkisi, literatürde yaygın olarak kullanılan bir kelime listesi öğrenme testi olan sözel bellek süreçleri testi (SBST), Wechsler Memory Scale (WMS) IV. alt testini oluşturan görsel bellek testi, dikkat ve uygunsuz yanıtı baskılama testi olan stroop renk-kelime testi ve kısa mental durum testi (KMDT) ile değerlendirildi. Elde edilen puanlar SPSS for Windows 10.0 paket program ile istatistiksel olarak analiz edildi. Bulgular: Denekler yaş, eğitim süresi ve menopoz süresi açısından karşılaştırıldıklarında gruplar arasında istatistiksel anlamlı fark saptanmadı. Tek ve çok değişkenli çok faktörlü istatistiksel analiz teknikleri, tibolon alan ve tibolon almayan olgular arasında, uygulanan test puanları açısından anlamlı bir fark olmadığını gösterdi. Sonuç: Çalışma sonucu elde edilen veriler, postmenopozal dönemde tibolon kullanımının sözel bellek, görsel bellek, dikkat ve uygunsuz yanıtı baskılama olmak üzere geniş bir kapsamda ele alınan bilişsel işlevler üzerinde olumlu etkisinin olmadığını göstermektedir.
ABSTRACT
Objective: There are several neurobiological mechanisms that might account for why tibolone may have a beneficial effect on cognition in postmenopausal women and reduce the risk of developing dementia. These mechanisms include effects on the estrogen alpha and/or beta receptors, modulation of transmitters, direct effect on neurons, lipoprotein alterations and prevention of cerebral ischemia. However, the results of the observational studies investigating the effects of tibolone on cognitive functions in postmenopausal women are still controversial. The aim of this study is to investigate the effects of tibolone on cognitive processes in healthy postmenopausal women without dementia. Material and Methods: Forty volunteer healthy postmenopausal women (20 receiving tibolone and 20 not receiving tibolone) from menopause outpatient clinic of department of Obstetrics and Gynecology took part in the study. Cognitive processes were measured through 19 scores obtained from a word list learning test; verbal memory learning test (SBST), a visual memory test; Weschler Memory Scale (WMS) IVth subgroup test, an attention and behavioral inhibition test; strop color-word test and mini mental state examination (MMST) test. All data were analyzed with the statistical package for the social sciences (SPSS) version 10.0 for Windows. Results: There was no statistical difference between the two groups according to age, education level and postmenopausal period. Univariate and multivariate multifactorial statistical analysis did not reveal significant differences between the tibolone and non tibolone groups. Conclusion: The results of this study revealed that treatment with tibolone in postmenopausal women did not result in better cognitive function (verbal memory, visual memory, attention and behavioral inhibition).
