ÖZET
Amaç: Preterm Erken Membran Rüptürü (Preterm EMR), günümüzde obstetrisyenlerin karşılaştığı en zor problemlerden birisidir. Amniosentezin, preterm EMRnin tanı ve yönetiminde ki, etkinliği değişik parametrelerle karşılaştırılarak araştırıldı.Çalışmanın Yapıldığı Yer: SSK Ege Doğumevi ve Kadın Hastalıkları Eğitim Hastanesi. Yenişehir- İzmir.Materyal ve Metod: Preterm EMR şüphesi olan, 26-36. haftalardaki gebeler çalışmaya dahil edildi. Çalışma kriterlerine uyan tüm gebelere, kendi onayları da alınarak amniosentez uygulandı. Amniosentezi takiben amniotik kaviteye indigo carmin boyası verildi. Amniotik sıvı glikozu bakıldı. Nitrazin testi, fern paterni, shake testi ve aerobik kültür ortamına ekim (International Microbio, Mycofast Evolution 2 kiti) uygulanarak sonuçlar karşılaştırıldı.Bulgular: Çalışma kriterlerine uyan 52 hastaya amniosentez uygulandı. Indigo Carmin Testi negatif olan grupta, pernatal mortalite, neonatal enfeksiyoz morbidite ve maternal morbiditeye rastlanmazken, pozitif olan grupta neonatal enfeksiyoz morbidite 10 bebekte görüldü ve bunların dört tanesinde neonatal ölüm gerçekleşti (%26.4). Her iki grup arasındaki farklılık istatiki olarak anlamlı idi (p<0.05).Sonuç: İntraamniotik enfeksiyonda, amniosentezle tanı konması geçerli bir yol gibi görünsede, kültürde üreme oluncaya kadar gebelerin büyük bir kısmı doğurmaktadır. Çalışmamızda, amniosentezi, EMR şüphesi olan gebelerin gerçek tanılarını bulmada etkili olduğunu saptadık ve bu sayede gereksiz bir çok morbid ve mortal olabilecek girişimlerin önlenebileceği kanaatine vardık.
ABSTRACT
Objective: Preterm premature rupture of membranes (PPROM) is one of the difficult problem in current obsteric practice. In this study the effectiveness of amniocentesis for the diagnosis and management of pPROM have been investigated comparing various parameters.Institution: SSK Aegean Maternity and Woman Teaching Hospital, Yenişehir, Izmir.Material and Methods: Pregnant women diagnosed as PPROM between their 26 and 36 weeks of pregnancy have been included in the study. Amniocentesis have been performed to all this pregnant women taking their permission. Indigo carmine dye is injected to amniotic cavity following amniocentesis. Amniotic fluid glucose levels were detected. Nitrazin test, fern pattern, shake test, and planting to aerobic culture medium (International Microbio, Mycofast Evolution 2 kit) were applied and the results were compared.Results: Amniocentesis were performed to 52 women suitable for the study criteria. Perinatal mortality, neonatal infectious morbidity and maternal morbidity were not detected in the patients which their indigo carmine tests were negative while neonatal infectious morbidity were seen in 10 newborn at positive group. Four neonatal deaths occured among these (26.4 %). The difference between these two gruops were statistically significant (p<0.05).Conclusion: Although amniocentesis is effective for the diagnosis of intraamniotic infection, most of the women would deliver until the positive culture have been detected. As a conclusion of our study, amniocentesis is effective for the true diagnosis of suspected PROM cases. Our opinion is that amniocentesis prevents unnecassary intervention which might be mortal and morbid for the mother and newborn.
