ÖZET
Giriş ve Amaç: Çalışmamızda; gebelikte artan konsantrasyonlarının, fetal gelişim üzerindeki etkisini saptamak ve gebelikte oluşabilecek komplikasyonları önceden tespit etmede, leptinin önemini tayin etmek amaçlanmıştır.Çalışmanın Yapıldığı Yer: Akdeniz Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum AnabilimdalıMateryel ve Metod: Çalışmada; normal gebelerde 2.trimestr, doğumda umblikal kord ve postpartum 1.gün maternal serum leptin konsantrasyonları spesifik RIA (Radioimmuno-assay) ile ölçülmüştür.Bulgular: Çalışmaya alınan 95 gebenin yaş ortalaması 26.8 ± 5.4 olup; 17 gebe sigara kullanan, 78 gebe sigara kullanmayan grubu oluşturdu. Gebelik haftasına göre 95 gebenin 84 tanesi term, 11 tanesi preterm doğum yaptı. Ancak doğumda gebelik haftasının tüm leptin değerleri ile ilişkisi istatistiki anlam taşımadı (p>0.05). Normal vaginal yolla doğum yapanlarla, sezaryan ile doğum yapanların tüm leptin değerleri arasında anlamlı bir istatistiksel ilişki bulunamadı (p>0.05). Çalışmaya alınan gebelerin maternal serum, umblikal kord ve doğum sonrası 1.gün maternal serum ortalama leptin düzeyleri sırasıyla 20.4 ng/ml ± 15.2 SD, 25.2 ng/ml ± 24 SD, 28.7ng/ml ± 25.1 SD olup, serum leptin düzeylerinin gebelik boyunca progresif olarak arttığı görüldü. Ayrıca DM (Diabetes Mellitus)lu ve preeklamptik gebelerde bu artış daha belirgin, IUGR (Intrauterin Growth Retardation)lı gebelerde daha düşük olarak saptandı.Tartışma: Bu çalışmadan elde edilen verilere göre, gebelikte preeklampsi, DM veya IUGR gelişimi maternal serum leptin düzeyi ile gösterilebilir; ancak bu daha geniş serilerde prospektif çalışmalarla ispatlanmalıdır.
ABSTRACT
Objective: Our objective was to investigate the impact of mid-pregnancy maternal serum leptin concentrations on the fetal growth and pregnancy complications.Institution: Akdeniz University Faculty of Medicine, Department of Obstetrics and Gynecology, Antalya.Material and Methods: Leptin plasma levels were measured by a specific RIA (Radioımmunuassay) in normal pregnant women, in the second trimester, after consecutive delivery from cord blood and in postpartum day one.Results: Mean age of 95 pregnant patients was 26.8 ± 5.4. 17 of 95 patients were smokers, the rest were nonsmokers. Delivery before 37 weeks was defined as preterm delivery. In our study, 11 patients had preterm delivery, 84 patients had term delivery. There was no statistical significant difference between pregnancy weeks and leptin levels when comparing the preterm and term delivery groups (p>0.05). We grouped the patients according to the route of delivery into spontaneous vaginal delivery and cesarean section. We found no statistical significant difference in leptin levels when comparing these two groups (p>0.05). The serum leptin concentrations during the mid gestation, cord blood and at postpartum were 20.4 ng/ml ± 15.2 SD, 25.2 ng/ml ± 24 SD, 28.7 ng/ml ± 25.1 SD respectively. Leptin levels increased progressively with pregnancy. The leptin levels were significantly higher in preeclamptic and diabetic patients compared with the mean level, lower in IUGR (Intrauterine Growth Retardation).Conclusion: Our data suggest that pregnancy maternal serum leptin levels may be helpful in detecting the subsequent development of preeclampsia, DM or IUGR. Further prospective studies are needed to confirm this.
