The Role of PUQE (Pregnancy-Unique Quantification of Emesis and Nausea) in Evaluation of the Indications for Inpatient Therapy in Pregnants with Nausea and Vomiting

ÖZET
Amaç: Bulantı-kusma, gebeliğin ilk aylarında en sık karşılaşılan sağlık sorunlarından biridir. Hastaların bir kısmı sıvı-elektrolit dengesizliği, şiddetli mide bulantısı, beslenememe gibi nedenlerden ötürü yatarak tedavi ihtiyacı gösterebilir. Klinik değerlendirmenin nesnel olarak yapılabilmesi için önerilen puanlama sistemlerinden Rhodes'in basitleştirilmesiyle hazırlananlardan biri de "Pregnancy-Unique Quantification of Emesis and nausea (PUQE)"dir. Gereç ve Yöntemler: Çalışmaya 01 Ağustos 2008 ile 30 Kasım 2008 tarihleri arasında hastanemize başvuran, bulantı-kusma yakınması olan, 14 hafta veya altındaki, canlı, tekil gebelikler dahil edilmiştir. Hastaların klinik ve laboratuvar değerlendirmesinin yanı sıra PUQE puanlama sistemi için anket de uygulanmıştır. Hastaneye yatan ve yatmayan hastalar PUQE puanı ve vücut kitle indeksi (VKİ) açısından değerlendirildi. Gruplar arası farklılıklar student-t testi veya ki-kare testi kullanılarak değerlendirildi. Bulgular: Çalışma döneminde kıstaslara uyan 68 hasta değerlendirmeye alındı. Hastaların yaş, VKİ ve gebelik haftası ortalaması sırasıyla 28.63 ± 5.05 (19-41); 24.52 ± 4.28 (16-35); 8.76 ± 2.13 (4-13)'tür. Hastaneye yatanlarla yatmayanlar arasında yaş, gebelik haftası, gravidite ve parite açısından farklılık saptanmamıştır. Hastaneye yatış endikasyonunun belirlenmesinde PUQE puanlama sistemi fizik muayeneyle karşılaştırılmasında duyarlılık %94.74, özgünlük %85.71, pozitif öngörü değeri %72, negatif öngörü değeri ise %97.67 olarak bulunmuştur. VKİ azalması ve PUQE puanının artması hastaneye yatış ile sıkı uyum göstermektedir. Sonuç: Gebeliğin ilk aylarında sık görülen bir sorun olan bulantı-kusmanın hangi hastada yatarak tedavi gerektirdiğini gösteren kesin kıstaslar bulunmamaktadır. Çalışmamızda PUQE testinin gebeliğe bağlı bulantı-kusmanın şiddetini değerlendirmede kullanılabilecek bir araç olduğu gösterilmiştir.
ABSTRACT
Objective: Nausea and vomiting is one of the commonest medical problem encountering during pregnancy. Some patients need to take care at the hospital due to fluid-electrolyte imbalance; severe nausea and being enable to eat. "Pregnancy-Unique Quantification of Emesis and nausea (PUQE) is one of the tests obtained by the facilitation of the Rhodes which is proposed for the objective evaluation of the clinical assessment. Material and Methods: The study was conducted from the 1st of August 2008 to 30th of November 2008 by evaluating the women who had singleton viable pregnancy less than 14 weeks of gestation with nausea and vomiting and who admitted to our institution. PUQE query was applied with the clinical and laboratory assessment. Women who treated in the hospital or not were compared in point of view of the PUQE score and the body mass index (BMI). Student-t test and ki-square tests were used for evaluation of the differences between the groups. Results: The results of 68 women who were eligible were investigated during the study period. The mean of the age, BMI and the gestational age were 28.63 ± 5.05 (19-41); 24.52 ± 4.28 (16-35); 8.76 ± 2.13 (4-13) respectively. The age, gestational age, gravidity and parity were similar between the women who treated in the hospital and who did not. When comparing the PUQE scoring system to the physical examination it was found the rate of sensitivity %94.74, the rate of specificity %85.71, positive predictive value %72 and negative predictive value %97.67. On the other hand lost on the BMI and increasing of the PUQE score was found in correlation with the hospitalization. Conclusion: There are not any strict criteria for determining the indication of hospitalization in women who suffer from the nausea and vomiting at the beginning of the pregnancy. Our study pointed that PUQE is a reliable tool for the evaluation of the severity of the nausea and vomiting of the pregnancy.
