ÖZET
Amaç: Bu çalışmada, fetal akustik stimülasyonun, fetal iyiliğin antenatal ve intrapartum araştırılmasında bir değeri olup olmadığını; diğer geleneksel testlere karşı üstünlüğü veya zayıf olan yönlerini araştırdık.Materyel ve Metod: Bu çalışma İzmir Atatürk Eğitim Hastanesi gebe polikliniğine başvuran veya doğum yapmak üzere doğumhaneye gelen gebeler arasında yapıldı. Gebelere önce 20 dakika kadar Nonstress test (NST) çekildi, daha sonra fetal akustik stimülasyon (FAS) yapıldı. Bu hastalar NST-FAS grubunu oluşturdu. Diğer yönden kontraksiyonları başladığı için başvuran veya bizim tarafımızdan indüklenen gebelere önce Kontraksiyon stress test (CST) daha sonra FAS uygulandı. Bu hastalarda CST-FAS grubunu oluşturdu. Toplam 77 hasta ile çalışma tamamlandı. Elde edilen sonuçlar Mc Nemer testi, Fischers exact test, Mann-Whitney test, Wilcoxon testi ile istatistiksel açıdan incelendi.Sonuçlar: FAS uygulandığında akselerasyon ve kombine yanıt veren fetusların tüm bulgularla sağlıklı fetuslar oldukları görüldü. Akustik stimülasyona, kalp hızının lakayıt olduğu veya deselere olduğu fetuslar ise daha düşük apgar skoruna sahip, problemli, kordon pHsı asidotik doğdular.
ABSTRACT
Objective: The aim of this study was to determine whether fetal acoustic stimulation (FAS) is meaningful in evaluating fetal health antenatally or intrapartum and also compare this method the advantages or disadvanages of this method compared with other tests.Study Design: The study group consisted of 77 patients referred to the out-patient clinic or delivery room of the Izmir Atatürk Educational Hospital IIIrd Dept. of Obstetrics and Gynecolodgy for antenatal care or for delivery. Non-stress testing was performed to all control patients followede immedately by FAS (NST-FAS group). Contraction stress testing (CST) was performed to all patients with active contractions (spontaneous or stimulated) followed immediately by FAS (CST-FAS group). Mann-Whitney test, Fischers exact test, Mc Nemers test and Wilcoxon test were used for statistical significance.Results: All fetuses with accelerations or combined reactions after FAS were found to be healthy newborns. Those fetuses without acceleration or with decelerations had low Apgar scores and low umblical pH values.
