ÖZET
Amaç: Uterin ve servikal patolojiler nedeni ile vakum küretaj yapılamayan 6-8 gebelik haftasında olan olgularda metotrexate ve misoprostol ile indüklenen medikal abortusun başarısı değerlendirmek. Çalışmanın Yapıldığı Yer: GATA Tıp Fakültesi Kadın Hastalıkları ve Doğum Anabilim Dalı, Ankara. Materyel ve Metod: Şubat 1995 - Ekim 1997 tarihleri arasında 6-8 haftalık gebeliklere sahip toplam 5 olguda, vakum küretaj girişimleri servikal ve uterin patolojiler nedeni ile başarısız kaldığı için 50 mg/m2 intramusküler (IM) metotrexate (MTX) ve 400mgr intravajinal misoprostol uygulandı. Bulgular: Bu olgularda saptanan uterin ve servikal patolojiler arasında 2 olguda submüköz myom, 1 olguda komplet uterus septus, 2 olguda ise servikal stenoz sayılabilir. Gestasyonel kese mevcudiyeti devam eden ve vajinal kanaması olmayan olgulara 24 saat sonra 400mgr intravajinal misoprostol uygulanması tekrarlanmıştır. Sonuçlar vajinal kanama ve 7 gün sonraki bhCG düzeyi ile değerlendirilmiştir. Tüm olgularda MTX ve misoprostol sonrası komplet abortus oluştu. 2 olguda bulantı ve kusma, 1 olguda ise diyare gelişti. Sonuç: Servikal ve uterin patolojiler nedeni ile dilatasyon ve küretaj (D&C) uygulamasının başarısızlıkla sonuçlandığı 5 olguda misoprostol ve MTX ile medikal abortusun etkili olarak uygulanabildiği görüldü.
ABSTRACT
Objective: To evaluate the success of methotrexate and misoprostol to induce medical abortion when uterine or cervical problems make suction curettage impossible between 6 and 8 weeks of gestation. Institution: GATA Medical School, Clinic of Gynecology and Obstetrics, Ankara. Materials and Methods: Five women, 6-8 weeks of gestation and with failed suction curettage due to cervical and uterine problems, were given methotrexate 50 mgr. per square meter intramuscularly and misoprostol 400mgr. suppository. Results: Five women had the following anatomic problems: submucous myoma in 2 cases, complete uterus septus in 1 cases, cervical stenosis in 2 cases. A repeat dose of misoprostol 400mgr. suppository was administered 24 hours later, if there was no bleeding and intrauterine gestational sac identified. Subjects were followed-up with serum bHCG on day seven, and vaginal bleeding. All cases had complete abortions with methotrexate, and misoprostol. Nausea and vomiting in two cases and diarrhea in one case were reported. Conclusion: We concluded those 5 women, with failed suction curettage due to uterine and cervical problems were induced medical abortion effectively with the use of methotrexate and misoprostol.
