Therapeutic Approaches in Ectopic Pregnancy According to the Focus Size
Ektopik Gebelik Olgularında Odak Boyutlarına Göre Tedavi Yaklaşımı

ÖZET
Amaç: Ektopik odak boyutunun, tanı kriterlerindeki ve tedavi yaklaşımındaki yerini değerlendirmek ve tartışmak. Gereç ve Yöntemler: Şişli Etfal Eğitim ve Araştırma Hastanesi Kadın Hastalıkları ve Doğum Kliniğinde Ocak 2007-Aralık 2011 tarihleri arasında, ektopik gebelik ön tanısıyla yatırılan; laparotomi, laparoskopi, tıbbi tedavi metotreksat (MTX) ve izlem tedavisi uygulanan 18-44 yaş arası tubal gebeliği olan 201 olgu odak boyutlarına göre retrospektif olarak değerlendirildi. Çalışma için etik kurul onayı alındı. Bulgular: Çalışmaya alınan 201 hastanın %60,2 (n=121)'sinin odak boyutu 3 cm ve üzerinde, %39,8 (n=80)'inin odak boyutu ise 3 cm ve altında idi. Ektopik odak boyutu 3 cm'nin altında olan %25 (n=20) hastaya tanı anında rüptür tanısı konuldu. Odak boyutu 3 cm ve üzeri olan %54,5 (n=66) hastaya tanı anında rüptür tanısı konuldu. Ektopik odak boyutu 3 cm ve üzeri olan olgularda tanı anında β-hCG değeri 5566±10428 mIU/mL bulundu. Odak boyutu 3 cm altındaki olgularda ise bu değer 1972±3364 mIU/mL olarak bulundu. p değeri p<0,05 olarak hesaplandı ve istatistiksel olarak anlamlı bulundu. 3 cm'den daha küçük odak boyutu olan hastalarda en sık uygulanan tedavi şekli MTX idi (%51).Tek doz başarısı %48,8 (n=20) ile istatistiksel olarak anlamlı bulundu (p<0,001). Sonuç: Ektopik odak boyutu 3 cm ve üzerinde olduğunda tanı anında odak rüptür ihtimali ve tanı anındaki β-hCG değeri anlamlı ve yüksektir. Ektopik odak boyutu 3 cm ve üzerinde olduğunda tedavi şekillerinden en çok laparotomi (salpenjektomi) uygulanmıştır. Tanı anında ektopik odak boyutu 3 cm altında olduğunda tek doz MTX tedavisi başarılı bir yöntemdir. Bu hasta grubuna izlem tedavisi yapıldığında da başarılı sonuçlar elde edilmiştir.
ABSTRACT
Objective: To evaluate and discuss the role of ectopic focus in diagnostic criteria and therapeutic approach. Material and Methods: Between January 2007-December 2011, in Şişli Etfal Education and Research Hospital, Department of Gynecology and Obstetrics Clinic, 201 patients treated with laparotomy, laparoscopy, medical [methotrexate (MTX)] and follow-up treatment, hospitalized with the diagnosis of tubal ectopic pregnancy, between the ages of 18-44, were analysed retrospectively accordig to ectopic focus size. Ethics Committee approval was obtained for his study. Results: 201 patients were included in this study. Focus size was 3 cm or more in 60.2% cases (n=121); focus size was less than 3 cm in 39.8% (n=80). 25% (n=20) of patients whose focus size was less than 3 cm, ectopic rupture was found at the time of the diagnosis. 54.5% (n=66) of patients whose focus size was 3 cm or more rupture was found at the time of the diagnosis. β-hCG values were found 5566±10 428 mIU/mL at the time of the diagnosis in patients whose ectopic focuses were 3 cm or more. In cases which ectopic size were 3 cm or less, this value was found 1972±3364 mIU/mL. p-value was calculated as p<0.05 and was statistically significant. In patients with less than 3 cm focus, the most common form of treatment was MTX (%51). The success of a single dose MTX treatment 48.8% (n=20) was found statistically significant (p<0.001). Conclusion: If the ectopic focus is 3 cm or more, the possibility of rupture and the β-hCG value at the time of the diagnosis is meaningful and higher. Laparotomy (salpingectomy) was the most common procedure in cases with ectopic focus were 3 cm or more. Treatment of a single dose MTX is a successful method when ectopic focus is less than 3 cm at the time of diagnosis. Successful results were obtained with follow-up treatment in this group of patients.
