ÖZET
Amaç: Akut karın tablosu oluşturan, süratli tanı konması ve tedavi edilmesi gereken torsiyone over tümörlerinin önemini belirlemek, preoperatif doğru tanı konmasının gerekliliğini ortaya koymak. Çalışmanın Yapıldığı Yer: SSK Tepecik Doğumevi ve Kadın Hastalıkları Hastanesi, İzmir. Materyel ve Metod: Çalışmaya son 14 yıllık süre içinde hastanemizde, akut batın ön tanısı ile opere edilerek, torsiyone over tümörü saptanmış olan ve herhangi bir nedenle laparotomi uygulanmış ve tesadüfen torsiyone over tümörü tanısı almış olan 71 olgu alındı. Hastaların yaş, parite, geçirilmiş adneksiyal operasyonları, reprodüktif bilgileri, başlıca yakınmaları, ağrının karakter ve lokalizasyonu retrospektif olarak değerlendirildi. Bulgular: Olgularımızın %77.5'inin 40 yaşın altında olduğu, torsiyon'un %66.1 hastada ovulatuvar ve postovulatuvar dönemde olduğu saptandı. Klinik bulgular; Hafif ateş, orta şiddette taşikardi, abdominal muayenede palpasyonda hassasiyet, rebound, orta derecede distansiyondu. Sonuç: Over torsiyonu çok sık değildir, fakat meydana geldiğinde tanı konmazsa çok ciddi komplikasyonlara yol açabilen bir durumdur. Erken tanı konarak opere edilen olgularda daha henüz nekroz gelişmediği ve venlerde trombüs oluşmadığından yalnızca torsiyon düzeltilerek tedavi önerilirse de, her an emboli risk nedeniyle lezyonun ekstirpasyonu tutulan yöntemdir. Preoperatif tanı kesin değilse laparoskopi düşünülmelidir.
ABSTRACT
Objective: To emphasize the importance of torsion of ovarian tumors which create a picture of acute abdomen and needs immediate preoperative diagnosis and treatment. Institution: SSK Tepecik Maternity Teaching Hospital, İzmir. Material and Methods: The study population was consisted of 71 patients in the last 14 years who were diagnosed as acute abdomen and found out to be torsion of ovarian tumor during operation or undergone laparatomy for other reasons but torsion of ovarian tumor was diagnosed incidentally. Patients age, parity, previous adnexal operations, reproductive information, chief complaints, localization and character of pain were evaluated respectively. Findings: 77.5% of patients were over 40 years of age, 66.1% of patients were in the ovulatory and postovulatory period. Clinical signs were slight fever moderate tachycardia, abdominal tenderness, rebound tenderness and moderate distension. Results: Although torsion of the ovaries is not seen frequently, if its not diagnosed accurately the complications are severe. Although during the early stage operations its recommended to restore the ovarian anatomy since the necrosis and the thrombosis has not developed yet, because of increased risk of embolism, extirpation is the preferred method. If the preoperative diagnosis is not exact laparoscopic evaluation should be considered.
